Home Health LPN Compensation

• The pay for this position is $33 - $36 per hour depending on experience
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health LPN Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required shift for this position is Monday - Friday until shift is complete. On-call required if requested
• Travel Area: Augusta

Home Health LPN Primary Functions:

• Primary function is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care.

Home Health LPN Primary Responsibilities:

• Under the direction of the RN, assists in providing nursing services and identifying the patient's physical, psycho-social and environmental needs
• Participates in planning, coordinating, and implementing care in conjunction with the RN, to provide quality, cost-effective patient care.
• Provide and ensure effective communication to patient/family, co-workers, and other health care professionals
• Assist the patient/family in learning appropriate self-care techniques and assess patient/family response to care.
• Provide care utilizing infection control measures that protect both staff and patient (OSHA).
• Assist the RN and/or physician in performing specialized duties related to end-of-life care for Hospice patients.
• Assists with admitting, discharging and transferring of patients.
• Possible on-call duties.
• Performs other related duties as assigned.

Home Health LPN Expectations:

• Ability to use electronic medical charting and time-keeping software (Homecare Homebase).
• Reliable transportation and current auto liability insurance coverage.
• Ability to recognize and maintain professional boundaries in relationships with co-workers, patient/family/caregiver.
• Ability to recognize an unsafe working environment and notify his or her supervisor immediately and attempt to safely correct the situation to the best of his or her abilities as soon as possible.
• Participate in annual in-service and performance improvement programs.

Home Health LPN Requirement:

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current Nursing License in the Practicing State
• Valid Driver’s License
• Current CPR Certification
• 2 years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Must have good and regular attendance.
• Must read, write and speak fluent English.
