Job Description Summary

PRN Licensed Master Social Worker (LMSW) Position

Job Responsibilities

• Assess patient/family social and emotional factors including the ability to cope with the illness
• Provide psychosocial counseling and support to the patient/family experiencing emotional, social and economic conflict
• Assist patient/family and staff in utilizing community resources
• Promote agency philosophy to ensure quality of care
• Assist patient/family financial and insurance status and assist with appropriate resources as indicated
• Participate in IDG meeting to establish, review and revise plan of care for each patient, including exchange of information, review of problems and sharing of professional support
• Support the patients and family’s unique cultural beliefs

Qualifications

• Master’s Degree in Social Work
• Experience in Home Health preferred
• Excellent written and verbal communication
• Ability to think critically and work as part of a team
• Ability to work efficiently in rapidly changing environments
