The Intake Liaison will be responsible for fielding intake calls and providing quality customer service to patients. The Liaison will enhance continuity of patient care by providing support to patients, liaisons in the field, and physician’s offices.

HOURS/SHIFT SCHEDULE: Full Time

Intake Liaison Requirements!

MUST be Competent with Hospital EMR/EPIC systems and ADOBE

· Ability to multitask, prioritize, and manage time efficiently, either independently or as an active member of a team

· Accurate and precise attention to detail

· Comfortable navigating and utilizing electronic medical records and computer programs

· Demonstrates effective verbal and written communication skills

· Excels at multitasking and is detail oriented

· Excels at time management and prioritization of the most critical tasks

· Goal-oriented, organized team player

· Knowledge of medical/healthcare and insurance terminology is highly preferred

· Maintains a positive outlook and approach to communication with clients, families, and other professionals

· High school degree or equivalent

· Possesses excellent verbal and written communication skills

· Preferably two years of experience in healthcare data entry

· Experience with home care admissions/intake/registration is ideal

· Self-starter individual

· Serves and protects Summit Home Care by adhering to professional standards, home care policies and procedures

· Strong computer skills in data entry, Microsoft Excel, and Word applications, must type 50 WPM

· Upholds a professional and respectful attitude in all aspects of the role

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

· Assists with home care intake coordination and patient relations

· Completes pre-surgery calls, responds to incoming intake calls

· Conduct screening and informational interviews with patients and their families as part of the admission process (ie. verifying patient information- demographics, billing payors, the method for consents, etc.)

· Enhances Summit Home Care’s reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to job accomplishments

· Enter client-specific information and communication notes into the electronic medical record system

· Navigating through hospital EMR’s/EPIC to gather client medical information and the necessary documentation to process a referral

· Provides customer service to patients by responding to queries of hospitals, attending physicians and their staff; sorting and distributing messages and documents; answering questions and requests

· Provides support to Business Development Team

· Records customer inquiries by documenting inquiry and response in customer’s accounts

· Resolves patient care issues by working one-on-one with liaisons and patients to standardize patient home care assessments; collecting relevant information; conferring with co-caregivers; speaking with patients about home-care needs.

· Send electronic consents for care to patients/POAs

· Track clients throughout their stay under our care, ensuring their satisfaction, and follow-up as needed

· Utilizing in-house referral trackers to effectively communicate with our Internal teams regarding referrals/patient care
