JOB SUMMARY:
Provide training and support to individuals with disabilities to maintain and enhance integrated community employment. Serve as a liaison between the individual and the employer to facilitate the necessary supports for the individual to become a successful employee.
ESSENTIAL FUNCTIONS:
- Provide job skills training and on-going supports to include completion of assigned job tasks, appropriate workplace social skills, problem solving, job adaptations, and other job- related skills.
- Analyze job duties and break down into manageable tasks to facilitate learning.
- Maintain ongoing communication with the employer, as needed, to address problems, concerns, or needs that may arise.
- Monitor quality of individual’s work to ensure it adheres to company’s standards and all tasks are completed within acceptable time frames.
- Facilitate development of natural supports for individuals receiving services to build and maintain positive working relationships with co-workers, supervisor, and customers.
- Complete daily documentation of progress notes and other reports in an accurate, timely and professional manner.
- Provide input into the development of the individual person-centered plan.
QUALIFICATIONS:
- Must be willing to work after-hours and/or on weekends, as required.
- Excellent interpersonal and communication skills
- Ability to project a professional image while interfacing with the community
- Basic computer skills to include keyboarding
- Must be able to work independently with limited supervision AND must be able to work in a team environment and produce results in conjunction with fellow team members.
Minimum Requirements
EXPERIENCE/EDUCATION REQUIRED:
- One year successful experience working with persons with intellectual and/or other types of developmental disabilities preferred
- High school diploma or equivalent required
- Valid driver’s license and safe driving record
