JOB SUMMARY:

Provide training and support to individuals with disabilities to maintain and enhance integrated community employment. Serve as a liaison between the individual and the employer to facilitate the necessary supports for the individual to become a successful employee.

ESSENTIAL FUNCTIONS:

  • Provide job skills training and on-going supports to include completion of assigned job tasks, appropriate workplace social skills, problem solving, job adaptations, and other job- related skills.
  • Analyze job duties and break down into manageable tasks to facilitate learning.
  • Maintain ongoing communication with the employer, as needed, to address problems, concerns, or needs that may arise.
  • Monitor quality of individual’s work to ensure it adheres to company’s standards and all tasks are completed within acceptable time frames.
  • Facilitate development of natural supports for individuals receiving services to build and maintain positive working relationships with co-workers, supervisor, and customers.
  • Complete daily documentation of progress notes and other reports in an accurate, timely and professional manner.
  • Provide input into the development of the individual person-centered plan.

QUALIFICATIONS:

  • Must be willing to work after-hours and/or on weekends, as required.
  • Excellent interpersonal and communication skills
  • Ability to project a professional image while interfacing with the community
  • Basic computer skills to include keyboarding
  • Must be able to work independently with limited supervision AND must be able to work in a team environment and produce results in conjunction with fellow team members.

Minimum Requirements


EXPERIENCE/EDUCATION REQUIRED:

  • One year successful experience working with persons with intellectual and/or other types of developmental disabilities preferred
  • High school diploma or equivalent required
  • Valid driver’s license and safe driving record
