Position: Intensive Case Manager

Schedule: Monday thru Friday 8:30am-5pm w/ on-call rotation

Location: Allentown, PA

Holcomb Behavioral Health Systems is seeking motivated professionals to join their team as an intensive case manager. You will carry a caseload of up to 30 clients, servicing areas within Leigh and Northampton counties. While in the role of an intensive case manager, you will develop strong skills to assess crisis situations and provide appropriate interventions where needed. You will gain an abundance of knowledge on resources available to those we serve by working in the community and partnering with various social service agencies.

You will be under the supervision of a Psychosocial Coordinator, whom you will work closely with to improve the lives of both adult and child consumers who face multiple obstacles in obtaining services that will improve their quality of life.

A typical day of an intensive case manager consists of meeting with the consumers on your caseload in all different environments. At times you’ll see consumers at home, within the community, in a public setting, or even when you support them at their mental health appointments. You will work hand in hand with housing agencies, homeless shelters, food banks, court systems, mental and medical health providers and others to advocate for your consumer. You will design a service plan with each consumer that utilizes SAMHSA’s 8 Dimensions of Wellness model to develop a goal plan that focuses on a holistic approach to the achievement and maintenance of wellness.
