General Description

Establish records for new persons served. File clinical documentation in records and monitor records on a regular basis to ensure that required documentation is submitted in a timely manner consistent with applicable standards.


Job responsibilities:


  • Complete eligibility determination screenings on persons served at point of intake, including initial diagnosis
  • Complete thorough psychosocial assessments to identify strengths, treatment needs, and any supports (natural or otherwise) of persons served
  • Ensure timely completion of intake documentation to meet agency and billing standards and expedite linkage to appropriate level of care
  • Determine needs of persons served by an assessment of areas of daily living and functioning to include: mental health, physical health, substance abuse, employment, housing, and skills in areas such as socialization, problem solving, coping, money management, and time management.
  • Create collaborative treatment plan with persons served in an effort to identify goals they wish to achieve while working with PFP
  • Inform persons served of expectations regarding services, focus on services and supports PFP can offer in achieving goals
  • With person served’s consent, consult with guardians, other professionals involved in care, family members and natural supports to enhance assessment and treatment goals
  • Willingness to meet persons served in the community for eligibility determination and assessment if necessary and clinically appropriate to reduce barriers to care
  • Assist persons served to reinvigorate a healthy system of emotional and social supports; provide a systemic intervention of helping families and others who may have once provided significant support
  • Coordinate correspondence with other agencies to obtain and provide client clinical records as needed
  • Maintain quality assurance records and files according to agency certification and accreditation requirements
  • Complete annual training hour requirements as outlined in the Employee Handbook
  • Perform other related tasks as assigned by the supervisor
