The Opportunity:
This Work Matters!
The purpose of this position is to manage and lead a program to facilitate connections between community and clinical sectors to directly improve the population health of the Immigrant/Refugee community. This position will work collaboratively amongst public health, health care, and community non-health care sectors to enhance systems, draft or update policies, establish a strategic place to produce better prevention and treatment outcomes for chronic diseases. In this role the incumbent will work collectively across Prevention and Health Promotion programs to address Multnomah County's leading causes of mortality and morbidity. The ability to center the lived experience of refugee/immigrant communities to advance the work is extremely critical in this role. Position requires advanced knowledge and understanding of the refugee and immigrant community.
This position will work in the Walnut Park Complex and will have a hybrid work schedule.
As the Immigrant and Refugee Systems Strategist, you will:
  • Support the Prevention and Health Promotion program in advancing foundational capabilities across cultural or ethnically specific communities as division representative and subject matter expert with the Immigrant and Refugee community
  • Develop, manage and present strategies, processes and methods for the Prevention and Health Promotion program to engage in coordinated, authentic and sustaining partnerships with the communities
  • Develop innovative, culturally relevant policy, system, and environmental implementation plans and processes that address identified barriers and support community-informed approaches
  • Provide technical assistance and training on implementation to providers, community leaders, company directors, etc
  • Track project plans per organization or provider, to document change processes, meet outcomes established per organization or provider based on client or community needs, and to meet funding requirements
  • Present change options to division, department and County leadership with data-driven supports to connect to County directives
  • Establish and maintain strategic partnerships within community and clinical sectors in the provision of physical and mental health services for newly arriving immigrants and refugees
  • Gather and lead inter-divisional and inter-agency programs, groups, and functions into a well-connected network of services and support offerings that may continually change based on political, military, resource or weather impacts, potentially resulting in immigration and/or refugee movement
  • Establish linkage of community leaders and service providers to individuals and families who share or demonstrate needs that mirror and support the client's culture, language, beliefs, etc. to gain solid connections that lead to the most appropriate care available
  • Develop and present community and public testimony on successes, challenges and barriers to be overcome in support of goals and strategic plan
  • Identify and map County work sites and community settings for service needs, service provision, etc. to include access needs and concerns by use of an equity lens
  • Conduct gap analysis for current service delivery within the Health Department, the County, community based and religious organizations, etc. to coordinate with and complement, not duplicate or overlap
  • Manage and coordinate contracts, MOUs, grant funding, data agreements, etc. for existing or new services or programs.

Minimum Required Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.
  • Education/Training: Equivalent to a Bachelor's Degree from an accredited college or university with major course work in public administration, business, statistics/math or a related field; AND
  • Experience: Five (5) years of increasingly responsible and major program development, policy analysis, or program compliance/monitoring and evaluation experience
Other: This position requires demonstrated professional and lived experience working with the refugee/immigrant communities and understands the specific health disparities that are commonly identified within newly arrived refugees and immigrants. This position will be responsible for facilitating connections between community and clinical sectors to improve population health of Immigrant/Refugee communities.
