POSITION SUMMARY: The IDT Technician is responsible for supporting the day-to-day operations of the interdisciplinary team (IDT). This position will provide support through tracking and documenting multiple functions of the IDT.

RESPONSIBILITIES AND DUTIES:

ESSENTIAL JOB FUNCTIONS:

  • Maintains the materials needed for IDT meetings, such as sign in sheets, agendas, and tracking information, and ensures they are prepared prior to IDT meetings
  • Attends and actively participates in IDT meetings
  • Manages the IDT meeting agenda; includes receiving agenda points from team members via a a variety of venues
  • Takes thorough and complete minutes for IDT meetings
  • Oversee monthly assessment lists and assessment delays
  • Coordinates supplementary meetings required by the IDT, including taking minutes and documenting in the electronic medical record (EHR)
  • Track participant status(es) such as hospitalizations, emergency department visits, subacute rehab stays, scheduled surgeries/procedures, and isolation precaution list; updates the agenda accordingly
  • Coordinates the interdisciplinary care plan schedule, including opening and closing care plans; updates care plans with attendance to ensure regulatory compliance
  • Provides administrative functions for the IDT, including maintaining email distribution lists
  • Tracks risk acuity scores and other data points for the IDT
  • Tracks information and enters documentation in the EHR to ensure regulatory compliance, related to IDT responsibilities of care plans, timely assessments, and service determination requests
  • Participates in Quality and Process Improvement meetings and activities as requested
  • Ensure IDT technician coverage for all IDT meetings
  • Other duties as requested/assigned

QUALIFICATIONS AND REQUIREMENTS:

  • High School diploma
  • Excellent written and verbal communication skills required
  • Excellent organizational abilities required
  • Experience working in a healthcare setting preferred
  • Experience working in an Electronic Health Record (EHR) preferred

Physical Requirements This job operates in an office environment and is largely a stationary position (over 50% of the time), however, some filing is required. This requires to the ability to move files, filling out paperwork and bending/standing as necessary. Operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer for nearly the entire work-day. Requires the ability to read paperwork, computer screens and communicate effectively through use of verbal and/or written forms. This role often is required to position oneself in a kneeling, bending, or crouching position to reach, install, or remove computer equipment. Requires the ability to move boxes, files, supplies/equipment up to 15 pounds using appropriate body mechanics.

Environmental Conditions This role operates in a closed office environment. Work-space may be shared. Working conditions may be noisy with fluctuating indoor temperatures. May be exposed to a risk of bodily injury through contact with moving instrumentation, substances and other conditions common to an office environment. This job requires working under stressful conditions. Moderate pressure to meet scheduled and recurring deadlines.
