JOB DESCRIPTION:

To Provide supervision and support to staff who are providing housing case management services and support to adult individuals.

ESSENTIAL FUNCTIONS:

  • Knowledge and demonstration of agency core values in day-to-day activities
  • Follows agency policy and procedures
  • Provides effective supervision utilizing agency standards, protocols, policy, procedures & evidence-based practices
  • Meet documentation timeliness standards; Supervisees meet same expectation
  • Demonstrates ability to effectively use technology and appropriate use of support
  • Completes annual trainings and updates
  • Coordinates services with other providers and collateral contacts
  • Maintains a smaller caseload
  • Assists staff and individuals in locating safe and affordable housing in an area of their choice
  • Develops individual recovery and service plans, objectives, and methods consistent with assessment of problem as appropriate
  • Extensive knowledge & ability to utilize various internal & external database & recordkeeping systems
  • Prepares reports, maintains files, participates in trainings, and provides statistical materials (outcomes)
  • Purchase with the individual household furnishings for their home
  • Assist staff as needed with obtaining benefits, collecting income, diagnostics and completing rent stipends to determine voucher per month
  • Maintain open communication with all treatment and service providers, families and any other supports or networks the individual is working within the community.
  • Assures that advocacy, linking, expediting and related activities are carried out as needed
  • Must have own transportation
  • Participation in team and community-based meetings
  • Oversight of Quality of Housing Services provided

SKILLS:

  • Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc.
  • Knowledge of Community Health Systems.

EDUCATION REQUIREMENTS:

  • Master’s Degree in Human Services or Related Field Plus 1 -Year Experience
  • Bachelor’s Degree in Human Services or Related Field Plus 3-Years Work Experience
  • Associate’s Degree in Human Services or Related-Field Plus 10- Years Work Experience

EXPERIENCE:

  • One – Ten years work-related experience dependent on education
