FUNCTION

This position is responsible for providing on-site direct case management services to residents in the Transitional Housing Program, as well as on-site services at the drop-in center. Evenings and weekends required.

TOP RESPONSIBILITIES

  • Assist clients in obtaining identification, medical care, employment, education, stable housing and make appropriate referrals.
  • Provide crisis counseling, mediation, and group activities.
  • Perform required written documentation.
  • Attend weekly treatment planning meeting, housing staff meeting, and supervision.
  • Develop relationships with NYC service providers, including but not limited to those addressing educational/vocational, medical, mental health, and housing needs.
  • Responsible for weekly reporting into contract databases.
  • Assist with Life Coach Program, including trainings, Life Retreats, and other duties as needed.

EDUCATION REQUIREMENTS

[x] High School

High School Diploma or equivalency required. BSW preferred.

TECHNICAL SKILL REQUIREMENTS

Computer knowledge and competent writing skills.

12pm-8pm Mond-Friday

PREFERRED QUALITIES

The Case Manager must be knowledgeable in the skills of therapeutic engagement and basic assessment. Experience working with adolescent/young adult population required. Knowledge of psychosocial needs of LGBTQ/homeless population is helpful. Transgender & gender non-conforming people are encouraged to apply. Bilingual (Spanish) speaking encouraged to apply.
