This Health Coach Consultant role is 100% remote and is part of the Disease Management team with Commercial Care Management at Aetna.

Candidates from compact RN states are preferred.

Normal Working Hours: Monday through Friday: 8:00am-4:30pm in time zone of residence for 3 days per week. Additionally, 12:30pm-9:00pm EST for 2 days per week. There is an additional late shift approximately every 4 weeks.

No weekends or holidays are required.
No travel is expected in this role.

Disease Management RN is responsible for engaging telephonically with members, to present actionable steps to address gaps in care, evaluate their medical needs and facilitate the member’s overall wellness.

The RN will address social determinants of health issues at root cause of care gap, facilitate care gap closure by providing education, connection to resources, and referrals to the member’s care management program for ongoing support.

The RN will develop, implement, support, and promote Health Services strategies, tactics, policies, and programs that drive the delivery of quality healthcare to establish competitive business advantage for Aetna.

Health Services strategies, policies, and programs are comprised of utilization management, quality management, network management and clinical coverage and policies.

The RN utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wrap around services to promote effective utilization of available resources and optimal, cost-effective outcomes.

Pay Range
The typical pay range for this role is:
Minimum: 58,760
Maximum: 125,840

Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.

Required Qualifications

• Must have an active and unrestricted RN license and, if located within the Nurse Licensure Compact jurisdiction, the ability to use their compact license without restriction
• Willingness and ability to obtain additional state licenses upon hire (paid for by the company)
• 3 years of medical-surgical experience in acute care/hospital

Preferred Qualifications

• Managed Care experience
• Coaching experience
• Bilingual in Spanish and English
• Current compact RN state licensure and resides in a compact state
• Telephone triage nursing experience.
• Ability to multitask on the computer using dual monitors and multiple programs
• BSN

Education
Associate Degree with RN licensure or equivalent experience
