  • Excellent computer skills (Microsoft Excel/Word)

  • Knowledge of community resources/experience setting up family engagement events & support groups/training.
  • Knowledge of HIPAA and Confidentiality Policies
  • Prefer Bilingual (Spanish/English)

Job Responsibilities:

  • Serve as first point of contact for all new potential students/clients
  • Follow up on all referrals within 48 hours
  • Review and document in Raintree all initial referrals and on-going status information.
  • Ensure full enrollment of school program including completion/submission of all information/paperwork required for classroom program.
    • Serve as Lead for Weekly Admission Team Meeting/ensuring weekly tracking log and Enrollment Planning Worksheet is updated.
  • Accurately initial input/scan all client demographic and school enrollment into Raintree database. Keep updated with current information at all times.
  • Schedule initial classroom tours/conduct tour as needed.
  • Assess family needs including gathering social-emotional information on the child and family/provide recommendations for families in acquiring needed resources for their children in the program.
  • Facilitate and ensure implementation of regular Parent Engagement activities including coordinating Title I (documentation, meetings, etc.).
  • Assist other staff members in problem solving on family issues/concerns.
  • Serve as the head of the Child Study team for campus/follow up/document any attendance issues.
  • Provide on-going support for families in the program, in collaboration with the Director of Child and Family Services
  • Work with Early Steps Family Care Coordinator and other referral sources to facilitate referrals for needed services.
  • Assist with community awareness/education programs to facilitate the marketing of services of UCP of Central Florida including regular visits to potential referral sources.
  • Provide monthly statistics on number of referrals, in-takes and current census of clients in program.
  • Provides additional support to campus as needed (i.e. ESE compliance assistance, scheduling, etc.).
  • Performs other duties as assigned.

Minimum Experience:

1-2 years case management experience

Prefer experience working in a school setting.

Education and Training:

Bachelor Degree in Social Work
