Salary Range: $30k-$35k

Hours Per Week: 21 hours per week

Join our Team! We have an exciting new opportunity for a Faith Based Community Outreach Worker- Part Time (21 Hours/wk.) for our mobile program.

Base Annual Salary Range: $30,000-$35,000. Retention Bonus of $3000 after 6 months of employment.

SUMMARY of DUTIES

MHA currently participates in a Westchester County Faith-Based Mental Health Initiative designed to equip clergy staff, faith, and Mental Health clinicians with the resources and guidance to foster hope and healing in their community. Our Driving Towards Resilience Program delivers community-based behavioral health services to individuals who are uninsured and underinsured or have otherwise been unable to connect to needed services as well as to members of the military, veterans, and their family members.

The Faith Based Community Outreach Worker is an individual who has direct experience with Faith Based communities, more specifically within the BIPOC communities, and has knowledge of the challenges experienced with Substance Use and/or mental health conditions and who assists with identifying and engaging individuals who may need services or support. The Faith Based Community Outreach Worker will employ a recovery-oriented and person-centered approach and have experience providing direct service and intervention to individuals with behavioral health conditions.

SPECIFIC DUTIES and RESPONSIBILITIES:

  • Assisting to educate the community about these new services.
  • Identifying and engaging with those who are eligible for MHA services.
  • maintaining contact to assure continuity of services.
  • connecting these individuals to other supportive services including, for example, housing, employment, educational support, physical care
  • Act as a resource for the Faith-Based Community, lending on-site support as needed.
  • Provide culturally sensitive outreach and engagement to anyone, anywhere.
  • Connect individuals with appropriate services, respecting rights of self-determination and choice.
  • Working collaboratively, identify other community services that support recovery and help establish connections to those services.
  • Maintain contact to assist individuals to negotiate systems, coordinate multiple aspects of care, and establish connections with additional service providers as needed.
  • Document client interactions in electronic health records to maintain a permanent record of client activity according to established methods and procedures.
  • Participate in agency training programs.
  • Follow the established agency/program policies & procedures.
  • Maintain appropriate professional standards.
  • Operate agency vehicle.
  • Other designated or related duties.

REQUIRED KNOWLEDGE and SKILLS:

  • The Faith-Based Community Outreach Worker will have direct experience working with Faith Based communities, providing direct service and intervention to individuals with Substance Use and/or Mental Health challenges.
  • An understanding or working knowledge of social determinants of health as it relates to health/mental health care and spirituality.
  • Knowledge of challenges experienced within faith-based communities as it relates to behavioral health care.
  • Knowledge of the challenges experienced by and issues pertinent to members of the military, veterans, and their family members is preferred.
  • Knowledge of the common challenges associated with substance use/mental health conditions.
  • Working familiarity with community resources, including entitlement programs, medical, financial, and legal services, housing and emergency food programs, and behavioral health treatment and rehabilitation services.
  • Understand the impact of culture on behavioral health conditions and be able to effectively engage clients in a culturally sensitive manner.
  • Experience with group work, community outreach led initiatives
  • Must be able to perform duties in an independent and efficient manner.
  • Familiarity with and ability to use an Electronic Health Record system.
  • The Faith-Based Community Outreach Worker is expected to practice in a recovery-oriented, person–centered manner.
  • Demonstrated commitment to diversity, equity, inclusion, and cultural humility.

QUALIFICATIONS:

  • The Faith-Based Community Outreach Worker will possess a minimum of a Bachelor’s Degree in Human Services (and or other applicable fields) and or hold a degree in Social Work, Mental Health Counseling, a Certified Alcohol and Substance Abuse Counselor (CASAC), and or Peer certification.
  • The Faith-Based Community Outreach Worker will have at least two years of experience working in this field.
  • Basic Computer knowledge and the ability to document in an electronic health record is required.
  • Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  • Must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
  • Must maintain current New York State Defensive Driving certification.
  • Must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
  • Must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
  • Must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
  • Must be and maintain clearance by the DCJS New York State Sex Offender Registry.
  • Must be legally eligible to work in the United States.
