Position Summary:

The Entitlements Coordinator is the first line of defense for the Community Services department in maintaining eligibility. Entitlements Coordinator files for ICF bed-reservations for lengthy hospital admissions and is responsible for interacting with the Fiscal Department on a regular basis around eligibility, admission, discharge, and other non-billable interruptions of service.


Essential Functions:


  • Educates family members on Medicaid, Social Security, food stamp and 3rd party insurance eligibility.
  • Coordinates screening and intake processes of residential applicants.
  • Coordinates service eligibility for the people we support, including admission, discharge, and other significant non-billable interruptions of service (i.e., extended hospitalizations, nursing home admissions).
  • Visits government organizations to submit applications and completes new Medicaid, Social Security, and food stamp, applications and recertification.
  • Assists parents with completing and processing guardianship applications.
  • Assists with Medicare Enrollment.
  • Serves as liaison between OPWDD, parents and private agencies.
  • Enrolls Residential and Waiver individuals in OPWDD Choices System.
  • Works with ACS to ensure continuation of benefits.
  • Process fair hearings and ensures that all benefits are active.
  • Advocates for the people we support and ensures due process.
  • Works in conjunction with agency fiscal department to ensure that benefits are active.
  • Ensures that program documentation follows mandates and agency protocols.
  • Other related tasks and duties as assigned.

Educational: BS in Human Services or related degree with strong experience with similar populations required.
