Job Summary
This position is responsible for providing needed support to consumers in their home and community include the following, although other duties may be assigned to meet the agency needs:

Essential Duties and Responsibilities include the following, although other duties may be assigned to meet Agency and family needs:

• Provides supervision, training, and monitoring of consumers according to their Individual Service Plan, High Risk Plans, and Behavior Support Plan.
• Provides training and support as needed in the area of hygiene by ensuring that consumers are bathed at least daily and teeth are brushed at least twice daily.
• Provides training and support as needed in the area grooming by ensuring that consumers are provided with choices and opportunities to care for their hair, nails, and other grooming needs.
• Provides training and support as needed in the area of dressing by ensuring that consumers are dressed appropriately for the weather and that clothing fits appropriately.
• Provides assistance and training to consumers to ensure the home is free of debris, clean, and safe from hazards.
• Provides assistance and training to consumers to ensure that healthy, well balanced meals are prepared and served according to consumer high risk plans.
• Provides training and opportunities for consumers to become integrated into their community.
• Implements and documents all goals as stated in the Individual Service Plan and Behavior Support Plan.
• Follows the Health Services Policy and Procedural Manual when providing support in medication administration and all other medical needs.
• Completes all paperwork accurately and in a timely manner to include, but not limited to, daily progress notes, Medication Administration Records, incident reports, and injury/illness reports.
• Works cooperatively with all staff, outside providers, and family members/guardians to ensure high quality services.
• Advocates for the wants and needs of the consumers.
• Ensures the care, welfare, safety, and security of all consumers.
• Follows all agency Policies and Procedures.

Education and Experience:

• Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
• Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
• Must be able to safely lift and carry at least 50 lbs.
• Must be able to safely bend, twist, push, pull and reach.

Job Knowledge, Skills and Abilities:

• Good verbal and written communication skills
• Ability to effectively work with coworkers and consumers
• Knowledge of working with individuals with disabilities
• Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
• High School Diploma preferred
• Must maintain a valid driver’s license and meet agency driving requirements

Pay: $14.00 - $16.00 per hour
