Cedar Hills Hospital is a 98-bed, freestanding psychiatric hospital and outpatient center specializing in mental health and substance use disorder treatment. Cedar Hills Hospital provides a wide range of services and programs that offer evidence-based treatment proven to have positive outcomes for our patients. Our inpatient and outpatient services offer supportive and compassionate care through specialty programs tailored to meet the needs of our patients.

Cedar Hills Hospital currently has an opening for a full time Discharge Planner. In this dynamic position, you will contact external case managers/managed care organizations throughout the patient’s stay to assist the treatment team in understanding the insurance company’s requirements for discharge planning. Responsible for having a thorough understanding of the patient’s treatment through communication with the treatment team. Advocates for the patient’s access to services during treatment team meetings and through individual physician contact. Coordinate the discharge plan for assigned patients.
Work Schedule: Monday – Friday, 8:30 am – 5:00 pm

Qualifications

Education: Bachelor’s degree from an accredited college or university in social work, mental health, nursing or related field preferred.


Experience: Two (2) year’s experience in a healthcare setting or managed care company with experience in patient assessment related to discharge planning, treatment planning, and/or case management.


Training: Successful completion of CPR, First Aid, Crisis Management and Service Excellence training must occur within 90 days of employment and prior to assisting in any restraining procedures. May be required to work occasional overtime and flexible hours.
