Summary: The Social Service Director implements and manages the overall daily functioning and programing in accordance with policy and compliance standards for the Social Service Department. Supervises the Social Services Department for the Center.

PRIMARY RESPONSIBILITIES:
1. Maintains working knowledge of company, Center, and the Social Services program, policies, procedures, and standards including the implementation of social/function-oriented model of patient care. Maintains compliance, quality control and delegates properly to obtain goals.

2. Manages and assumes authority, responsibility and accountability for the department.

3. Supervises the Social Workers duties and responsibilities to assure compliance with Center policy and procedures, and the regulations that govern long term care.

4. Recruits and onboards new Social Workers hires, conducts performance evaluations and educates Center Social Workers.

5. Ensures the delivery of compassionate quality of care approach to patients and ensures their care needs are met.

6. Is a member of the interdisciplinary team in the development and maintenance of the social service and activity portions of the plan of care for each patient.

7. Communicates with other disciplines to evaluate patient responses to interventions, suggests alternative approaches, and revises and updates plans of care to reflect progress towards goals.

8. Performs daily rounds of the Center to inspect the care environment, evaluate staffing needs, and monitor compliance

9. Acts promptly to resolve issues directly with staff members and/or reports relevant concerns to CEO/Administrator.

10. Promptly reports problems, significant changes in condition, injuries, and family dissatisfaction to nursing services and provides timely responses to inquiries of same.

11. Assists team to effectively manage patient behavioral issues and interventions.

12. Responds to questions and issues identified by patients/residents and families to determine satisfaction with services.

13. Provides timely and appropriate charting, documentation, and record keeping.

14. Develops patient's daily, weekly, and monthly schedules.

15. Develops and maintains Family Resource Center on-site.

16. Attends facilitator meetings and other pertinent educational programs as approved.

17. Other duties as assigned

EDUCATION

1. Master's degree preferred in a related field required (psychology, social work, sociology, recreational therapy)

2. Requires licensure in health care field per WV state code

3. Minimum of three years human services experience with two years working with dementia or Alzheimer's care/patients preferred
