We are currently seeking an exceptional and energetic individual with an outgoing personality to join us as our Memory Care Director for our Tuscan Gardens of Palm Coast Community. As the Memory Program Director, you will oversee the needs of our Memory Care and Enhanced Assisted Living (EAL) Residents within our Tuscan Gardens Aspire and Bridges Programs.

The Memory Care Director ensures the safety and well-being of our Memory Care and Enhanced Assisted Living Residents. The Director will be designing and executing a thriving program for our residents in memory care as well as our Bridges Program for our EAL residents. The position requires the ability to plan, organize, develop, direct, and evaluate Memory Care/EAL services ensuring the highest degree of quality resident care and memory care/EAL program. These functions include:

  • Create, implement, execute and manage, the Aspire Memory Care Program for Tuscan Gardens.
  • Create, implement, execute and manage the Bridges Enhanced Assisted Living Program.
  • Manage and direct functions of the activities programming in the Memory Care/EAL departments to include ongoing staff training and program implementation.
  • Direct Memory Care/EAL staff in the planning and coordination of the programming functions in conjunction with the Health Services Director.
  • Evaluate resident activity needs and work in collaboration with Memory Care/EAL staff members to explore opportunities for the resident engagement, plan, and schedule all daily activities based on individual needs and resident interest.
  • Observe and evaluate performance of Memory Care/EAL staff to ensure quality standards and services. In conjunction with the Health Services Director, assist with interviewing, selecting, and training staff involved with providing activities.
  • Oversee and coordinate hands-on recreational activities including current events, discussion groups, re-motivation therapy, sensory stimulation, cultural activities, music therapy, religious services, special entertainment, outing trips, special events, and one-on-one visits, as indicated.
  • Provide education, training, and support groups on the topic of dementia and Alzheimer’s to staff and resident’s families.
  • Evaluate programs, operations, and training in order to maintain the highest possible level of service delivery.
  • Meet all new residents upon admission and orient them and their families to the programming aspects of Memory Care and/or EAL.
  • Coordinate family involvement in the activities and special events.
  • Ensure necessary supplies and resources are on hand for each program. Purchases and inventories supply and equipment.
  • Direct volunteer activities in Memory Care/EAL, enlist their support and coordinate their efforts; provide orientation, training, and supervision of volunteers.
  • Maintain accurate documentation of resident progress and responses to program.
  • Review and address the concerns and suggestions of residents and family members and resolve customer service issues in a timely manner.
  • Embrace the person-centered care model that empowers staff to keep the individual at the center of the care planning and decision-making process.
  • Ensure that residents are supported in achieving the highest level of physical, mental, and psychosocial well-being.
  • Participate in community committees as required.
  • Attend in-service training and education sessions, as assigned.
  • Maintain appropriate confidentiality of information.
  • Implement appropriate behavioral interventions as needed for Memory Care/EAL residents.
  • Support Health Services Director in overseeing care associates.
  • Support Culinary Services Director in overseeing dining associates.
  • Support Environmental Services Director in overseeing housekeeping associates.

Requirements:

  • Assisted Living CORE License required
  • Minimum Associate Degree required, bachelor’s degree preferred.
  • 2-5 years experience with hands-on Memory Care activities required.
  • 5 years experience in an Assisted Living/Memory Care facility required.
  • Knowledgeable in creating Memory Care activities calendar.
  • Teepa Snow certification or other equivalent strongly preferred.
  • Knowledge of music therapy preferred.
  • Substantial knowledge of memory care required.
  • General/medical knowledge and application.
  • Ability to respond to medical emergencies.
