Position Summary:

The CPCP is responsible for creation, maintenance and training on any and all documents to secure services and funding for the people supported by KCI. This requires the ability to someone to be able to create and form meaningful relationships quickly, act as a liaison between loved ones and KCI, be detail oriented, ensure all necessary information is communicated appropriately and facilitate meetings. This position is also responsible for training on and compliance with PCPs.


Essential Responsibilities:

  • Maintenance, contributing to, and training on the person-centered plans (PCP) for every person served by KCI.
  • Advocating for the people supported by KCI to receive the support they need to reach their fullest potential and live their richest lives.
  • Ensuring that all services on the PCP are aligned with current waiver services.
  • Promoting independence and self-advocacy for the people we support to contribute to or run their own PCP.
  • Submit all necessary paperwork for funding (MSFPR or SSP) and planning (PCP) to DDA.
  • Ensure that all necessary services are funded and to ensure that all services that are delivered are identified as funded by DDA correspondence.
  • Enter and investigate all incident reports and hold all meetings involving incident reports.
  • Liaison between KCI and DDA or loved ones of people we support.
  • Other duties as assigned.


Minimum Qualifications:

Education:

  • High school diploma or equivalent
  • Preferred – college degree

Experience:

  • Required- Minimum of 1-year experience in a technology role or comparable setting


Physical requirements:

  • Ability to lift 50 lbs.


License/Certifications:

  • none
