The Contact Tracer shall provide assistance with administrative activities associated with Public Health emergencies, such as but not limited to, COVID-19 or any other communicable diseases.
- Gather information related to all cases that meet the criteria for COVID-19 cases
- Evaluate information related to all COVID-19 cases based on CDC protocols for internal response actions.
- Trace cases for known/identified close contacts and make notifications to close contacts.
- Provide approved information regarding facility access and workforce guidance for individuals that meet the criteria for COVID-19 cases
- Refer individuals to testing according to agency protocol.
- Notify CDC Facilities for cleaning/decontamination based on COVID-19 protocols.
- Other duties as assigned
- Diploma in practical nursing through and approved educational program OR Associates Degree
- Possess an active and unrestricted license as a LVN or LPN OR Contract Tracing Certification
- Minimum 2 years of experience as a LVN or LPN OR Contact Tracing
Knowledge, Skills and Abilities
- Experience using Microsoft applications
- Ability to communicate effectively both orally and in writing
- Ability to multi-task
- Related experience or customer service oriented professionals with experience of frequently interacting with customers
- Have in-depth knowledge of CDC guidelines
