As a Complex Care CNA, you will provide clinical mental health personal support services, housekeeping and laundry services, food shopping, and transportation in coordination with multi-disciplinary team for clients with complex care needs.

ESSENTIAL RESPONSIBILITIES/TASKS:

  • Utilize comprehensive nursing assessments, plans, interventions and evaluations in direct client care.
  • Monitor medications, teach clients effective techniques for managing medications and assist clients in adhering to prescribed regimens
  • Monitor signs and symptoms of psychiatric functioning. Consult with Registered Nurse other treatment staff
  • Take vital signs and monitor patients for bruises, other injuries/wounds
  • Collect information about conditions and treatment plans from caregivers, nurses and doctors
  • Assist with meal preparation, serving and monitoring of nutritional status
  • Coach and assist with light housekeeping and maintaining hygienic living environment
  • Light laundry duties; Maintain/change clean bed linens
  • Assist individuals in transferring from bed to commode to chair
  • Turn or reposition patients who are bedridden
  • Collaborate with other providers, agencies and individuals in the client's network of care
  • Coordinate services within the Organization as needed
  • Assist clients in developing and strengthening their support network to accomplish goals and objectives

Education/Experience:

  • Certified Nursing Assistant credentialing and experience required
  • Minimum (1) year experience in a mental health setting working with clients with disabilities preferred
  • Experience or training in related state and federal assistance programs also preferred
  • Basic computer literacy skills are necessary

Certifications:

  • Rhode Island certified nursing assistant certification required
