Job Scope: Actively contribute as a clinician (case manager or counselor) working on a community based team providing intensive, holistic and recovery oriented behavioral health and co-occurring substance use services to persons living with serious and persistent mental illness who desire to live independently and engage fully in community life. Community Support Team (CST) counselors and case managers provide a range of services which may include assessment and service planning, crisis intervention, supportive counseling, community support services, and case management to a shared caseload of adults to target assessed areas of functional impairment and promote independence through the development and use of natural community supports. Services are provided in person’s homes and other community spaces during flexible daytime, evening, and weekend hours to best support individual’s success.
Proof of COVID-19 vaccination required. Accommodation for medical and religious exemption will be considered. Weekly testing required for approved vaccination exemptions.
Education, Experience and Other Requirements
  • Valid IL Driver’s License, a driving record that meets organizational policy, and daily access to a well maintained/insured personal vehicle in order to provide community-based services to clients.
  • HS Diploma/GED or Associate’s Degree with at least 6 months experience working with individuals with serious and persistent mental illness providing a variety of clinical interventions including case management and community support. Preferred Bachelor’s Degree from an accredited college or university in social work, psychology or a related discipline.
  • Applicants for case manager or counselor with a Masters Degree from an accredited college or university in social work, psychology or a related discipline strongly encouraged to apply; licensure supervision provided
  • Preferred experience working with individuals with serious and persistent mental illness providing a variety of clinical interventions including assessment, case management and community support
  • Preferred exposure to working in non-traditional treatment environments i.e.in-home, residential, hospital, nursing facility, and community settings
  • Basic computer proficiency, knowledge of Word and Internet skills, smartphone messaging and applications, and business communication platforms
  • Persons with CRSS/NCRS and/or CADC certifications strongly encouraged to apply
  • IM-CANS certification encouraged
  • Exempt, Full-Time, 37.5 hour work week
  • Must work a flexible schedule in response to the needs of the target population served, including some evening and rotating Saturday hours.
