Floating Community Support Specialist Overview:

The Floating Community Support Specialist at ReDiscover will provide assistance to people living with a substance use disorder on their recovery journey to live successfully in the community. The Community Support Specialist can expect to be a vital asset to ReDiscover clients by utilizing:

Key Functions:

• Facilitating each client's achievement of long-range outcomes of symptom management, employment, and independent living based upon the needs of the clients.
• Engages difficult-to-serve clients by investigating all potential resources in order to gain access and make connections.
• Assesses clients completely using all the assessment tools required to develop a comprehensive assessment.
• Ensures that treatment plan objectives, interventions, services provided, and documentation are related to needs identified within the assessments and are medically necessary.
• Routinely identifies warning signs and symptoms, responds to the changes and educates clients on symptom management.
• Identifying and providing clinical interventions and referrals for clients based on their needs.
• Empowering clients to develop and nurture the knowledge, skills, and support systems crucial to promoting optimal functioning in the community
• Continually assess clients for lack of progress, progress, or emergent needs and address those needs clinically.

• Bachelors Degree from an accredited college or university in a social science related discipline. Qualifying experience may be substituted for education.
• A working knowledge of mental illness.
  One year of direct service to the target populations or other related human service experience preferred.
• Good written, oral, and interpersonal communication skills.
• Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite.
• High integrity by observing ReDiscover policy/procedures, accreditation standards, laws, and ethics.
• Self-driven individual with a customer service focus.
• Team-oriented and skilled in collaborating with others.
• A valid automobile operators license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours.
