Jefferson Parish Human Services Authority (JPHSA) is a Local Governing Entity (LGE) created by the Louisiana Legislature in 1989. JPHSA is responsible for the management and operation of behavioral health and developmental disability Services in Jefferson Parish. JPHSA provides fully integrated primary and behavioral healthcare services through its JeffCare program.
The Developmental Disabilities Community Services (DDCS) division provides services and supports to assist individuals with intellectual and/or developmental disabilities to live in the least restrictive environments and to participate as fully as possible within their communities. The Community Service Professional 3 (Personal Support Coordinator) works in the DDCS division under the direction of the Community Service Supervisor. The incumbent is responsible for providing Support Coordination to individuals with intellectual and/or developmental disabilities and their families.
40% Monitoring, Planning, and Documentation
- Develops a Comprehensive Plan of Support (CPOS), Request for Services (RFS) if applicable, and assists the individual/ family choose a service provider needed according to person-centered planning principles and best practices for CPOS development and implementation.
- Ensures the CPOS is completed electronically within two business days following the meeting with the individual/ family.
- Informs the individual/ family of the funding decision on the day of the decision for urgent RFS and within seven business days of the decision for non-urgent RFS.
- Develops, implements, and monitors individual and family support budgets and contracts by doing the following:
- Preparing individual/ family support contracts and meeting with the individual/ family to review and obtain the required signatures.
- Monitoring utilization (under or over) of funding and service delivery of contracted services to ensure they are responsive, flexible, person-centered, reflect best practices and are satisfactory and beneficial to individual/ family.
- Collecting data that supports the individual/ family use of funding as described on the CPOS.
- Ensuring contract invoices expenditures are within approved budget projections.
- Reviewing and monitoring contract invoices and expenditures timely before recommending approval for payment by Division Director.
- Amending contracts as needed to reflect changes in individual/ family support needs throughout the fiscal year.
- Providing corrective actions for invoice approval resolution.
40 % Support Coordination
- Provides Support Coordination to up to 65 individuals/ families monthly.
- Provides direct support and assistance advocacy as needed to ensure that the individuals/ families are connected to community resources identified on the CPOS.
- Provides advocacy, education, and follow-up as needed for school-age individuals and their families to address educational and vocational goals.
- Attends interagency transition meetings (as needed) and IEP meetings with Jefferson Parish Public Schools, Families Helping Families of Greater New Orleans, and Louisiana Rehabilitation Services.
- Advocates on behalf of the individual/family with the Family Support Committee (FSC) when DDCS funding is needed to meet the priority of need addressed by the CPOS.
- Works closely with the individual/family to ensure the CPOS is effective, satisfactory, and assists in avoiding institutionalization or out-of-home placement and unnecessary psychiatric hospitalization when possible.
- Develops and maintains knowledge of available (and evolving) community resources to ensure individuals/ families are fully informed of community resources, which may benefit them.
20% Quality Assurance
- Participates in quality assurance activities, including gathering individual/ family support services information for the quarterly chart audit.
- Conducts quarterly service monitoring with individuals/ families receiving supports and services through either face-to-face or telephone contact by the end of the last day of each quarterly reporting period.
- Conducts monthly home visits to individuals utilizing supported living services.
- Corrects and updates demographic information in the Office for Citizens with Developmental Disabilities (OCDD) Participant Services database as needed.
- Ensures each individual's service record contains the CPOS, correspondence to and from the individual/family, and additional documents related to Individual Family Support (IFS) RFS such as Prioritization for IFS funding tool and Notice of Right to Appeal.
- Ensures all documentation related to services and the individual's health and safety are filed appropriately in the service record on or before the 15th day of the month following the month's close.
- Ensures that the progress notes are completed electronically, filed in the individual's record on or before the 15th day of the month following the month's close.
- Performs other duties as assigned to meet DDCS and/or JPSHA needs.
Physical Requirements
This position is mostly a sedentary role with occasional light lifting and bending; lifting up to ten pounds is required. Work activity includes operating a computer, printer, phone, and a copy machine and accessing file cabinets. Work activities occur inside and outside, resulting in exposure to conditions such as heat, cold, and dust.

The maximum salary for the Community Service Professional 3 is $80,267.
