Qualifications: Bachelors Degree in Human Services, or related program with one year of supervisory experience preferred; OR AA/AS Degree in Human services or related program and three years of experience in a vocational setting; OR High School Diploma and five years of experience in a vocational setting and 1 year of supervisory experience required. Experience working with people with disabilities is preferred.


Work Authorization/Security Clearance:
Authorized to work in the United States and pass all background clearances.


Summary/Objective:-
Responsible for each assigned persons we support vocational plan of service and required program documentation, including forms and regulations for SourceAmerica, NYSID, Medicaid Waiver, Department of Labor. Assist persons we support in the Work Center Program and in the Community Prevocational (CPV) Program to develop work skills and behaviors in order for them to reach their maximum vocational potential. Provide daily interventions with persons we support and work in cooperation with all program staff to promote Independence, Integration, Individualization and Productivity. Responsible for the complete oversight of the Community Prevocational Program to include any regulatory changes and/or updates. Responsible for the supervision of the Community Prevocational Professionals to include completion of annual evaluations and corrective actions as needed. Responsible for all intakes for the Community Prevocational Program. Responsible for the oversight and implementation of staff training. Keep the Administrator of Community Based Employment & Community Prevocational services informed of any concerns and/or updates.


Essential Functions:

  • Schedule team meetings and complete all documentation required for annual reviews, semi-annual reviews, and interim meetings.
  • Complete CPV, Prevoc ISP programs, annual and semi-annual reviews, habilitative plans, monthly notes, and six month reviews of progress on Therap. Complete T-logs and PSU data entries as necessary.
  • Complete ISP Plans and Personal Focus Worksheets as necessary in Therap.
  • Complete discharge documentation for sheltered employment program and admission documentation for Community Prevocational Services.
  • Identify and help address barriers that may hinder persons we support progress toward supported employment.
  • Monitor waiver Prevoc services to ensure accuracy and completeness for billing. Complete all required documentation for monthly billing.
  • Assist persons we support to gain access to needed community services and supports.
  • Maintain communication with families, MSCs and all voluntary agencies.
  • Complete DDP1 & 2, needs assessments and annual review/revision. Complete DDP1 Supplement Form in Choices.
  • Participate in on-call rotation for Investigations of Reportable Incidents and Allegations of Abuse.
  • Responsible for developing CPV daily and weekly schedule of Community Prevocational activities for persons we support and CPV staff and make adjustments accordingly. Ensure that all volunteer agencies are aware of any changes.
  • Assist with obtaining and establishing volunteer activities in the community.
  • Responsible for maintaining contact with volunteer agencies to ensure satisfaction of services.
  • Ensure that coverage is maintained in the absence of Community Prevocational Professionals.
  • Develop and implement Community Prevocational classroom activities for people we support.
  • Attend and satisfactorily complete all required in-services.

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
