Community Liaison Specialist
Remote/Hybrid Employment
Sevita Adult Day Health is seeking an Outreach Representative to serve as a liaison between our participants, their families and the community; enhancing community relationships, improving customer services/programs, and promoting a positive public image of the program. This position is responsible for raising awareness of the program and for developing and managing the pipeline of referrals for Adult Day Health (ADH) Program
Responsibilities may include but are not limited to:
- Collaborating with Strategic Development Director to create and implement strategic sales and marketing plans and community outreach efforts to support growth
- Serves as a sales and marketing representative for designated locations to support expansion and growth of clinical programs and on behalf of the company
- Serve as a point of contact between the organization and the community, along with potential new individuals served, families and referral sources to increase quality referrals and enrollments
- Work with center/program staff to coordinate pre-enrollment process and to insure that prospects fully understand criteria and program offerings
- Develop weekly/monthly outreach plans whereas the result is to engage with decision makers and referral generators
- Assist potential new participants and his/her family with eligibility issues
- Identify, develop, and foster relationships with the community, referral sources, and other community groups
- Participates in business development and strategy planning sessions
- Develop, plan, and/or coordinate various marketing activities within the center(s) as well as within the community
- Assists in maintaining company branding/image standards
- Attend various networking events and participate in professional networking groups, etc.
Qualifications:
- Bachelor's / Associates Preferred
- 3-5 years’ experience in a similar type of role
