Job description
The Community Health Worker (CHWs) shall serve as front-line health workers who come from the communities they serve which aid in building trust and vital relationships with the residents of the community. This trusting relationship enables CHWs to be effective links between their own communities and systems of health care and social services. The goal of the CHW is to lower health disparities in their community by providing access to services, improving the quality and cultural competence of care, creating an effective system of chronic disease management, and increasing the health knowledge and self-sufficiency for clients.The Community Health Worker works from home and travels to and from various locations to meet with clients and to attend meetings and other engagements on Hawaii Island. May be required to work evenings and on weekends to meet the needs of the agency.
