Opportunity Overview

North Shore Community Health (NSCH) is an industry leader in exceptional delivery of community healthcare. We are looking for a Community Health Worker for Community Programs (CHW – CP). U Reporting to the Director of Community Programs, The Community Health Worker (CHW) for Community Programs is an advocate for both NSCH’s patients and the communities served by NSCH. As a member of the Community Programs team, the CHW is an essential member in the implementation and execution of programs that support NSCH’s mission to build healthy communities. As a team member at our health center, you will work with staff who are racially, ethnically and culturally diverse, so being culturally competent is required. A sense of humor is also very important!

Duties/Responsibilities:

Community Advocacy and Outreach

  • Develop relationships with community leaders and members
  • Implement community programs and interventions
  • Attend community events to promote and educate on NSCH’s services
  • At the direction of the Director of Community Programs, assist in gathering data regarding gaps in health services in communities served.
  • Identify community resources that provide needed support for NSCH patients
  • Educate NSCH staff about community characteristics, resources and needs

Patient Navigation

  • Identify community members that do not have a medical home and assist with establishing care
  • Assist NSCH patients in navigating to needed health care and community services such as food assistance, housing assistance and legal services
  • Collaborate with population health coordinators to identify NSCH patients that require community outreach and intervention
  • Assists individuals in enrolling in health insurance

Health Education

  • Provide health education to individuals, community members and groups on a variety of health prevention and wellness promotion topics
  • Provide administrative support for Community Health RN Manager at provision of health services in the community
Requirements:
  • High school diploma or equivalent
  • Knowledge of communities served by NSCH
  • Certified Community Health Worker preferred
  • Experience in community outreach and program support
  • Excellent customer service, communication and organizational skills
  • Community Health Center experience preferred
  • Certified Application Counselor (CAC) encouraged
  • Proficient in technology (Electronic Medical Records, Microsoft Office)
  • Evening and weekend hours required.
  • Bilingual Spanish/Portuguese preferred
  • Ability to work in a culturally-diverse environment.

As a health center, NSCH expects all employees to receive all required vaccinations and health screenings each year.
