Position:

North Central District Health Department is searching for a full time Community Health Worker. This is a time limited contract position and serves under the direction of the Health Education Director. The salary for this position is $22.00 hourly. Some of the responsibilities for this position include, but are not limited to;

· Participate in community coalitions and other community groups planning goals and activities to address health priorities of a community with focus on minority outreach.

· Work with others to facilitate completion of planned activities.

· Make presentations to groups on topics related to community health projects.

· Review publications and other appropriate materials and sources for further opportunities.

· Organize community groups for the promotion of health services and participate in community group discussions in support of improved personal and environmental health practices.

· Contacts and build relationships with organizations, agencies, individuals, and others in the community to form partnerships to address health priorities.

· Participate in medical services clinics.

· Administer screening tests and measures.

· Participate in the development of training and train other community health workers and community volunteers giving assistance in the conduct of medical services clinics.

· Research for existing statistics on health indicators and participates in data collection when statistics are not available.

· Record all activities and services, record test results and prepare documents for data tabulation and analysis.

Minimum Education, Training or Experience:

• High School Diploma or GED and three (3) years’ experience in public health or in another position with the provision of social services, supportive services, personal care, working within a harm reduction approach or related duties or where the skills, knowledge and other abilities can be demonstrated. At least 3 years of experience in coordinating and organizing community outreach activities, related to minority populations preferred.

North Central District Health Department serves the communities of Henry, Shelby, Spencer, and Trimble counties in Kentucky. We are centrally located between the two largest cities in Kentucky, Louisville, and Lexington. We serve rural and small urban like communities.
