Trilogy is seeking a full-time Behavioral Health Nurse to join our team. We are looking for an individual who is caring and compassionate, has a solution-focused mindset, excellent communication skills, and is an advocate for the wellbeing of patients. Responsibilities:

  • Make preliminary assessments of clients’ health problems based on medical history and diagnostic results and prioritize for medical attention.
  • Address health needs of clients with serious mental illness and substance use disorders; i.e. administer specified medications, including long-acting injectable antipsychotics, obtain vital signs, point-of-care testing (POCT), GeneSight specimens for genetic testing, and document in the electronic health record
  • Collaborate with clinical and non-clinical staff on client care plans and ensure quality standards are met. Evaluate effectiveness, monitor progress, and effect change as needed.
  • Attend daily ACT meetings to provide updates to team on client’s current needs
  • Attend agency wide nursing meetings as scheduled
  • Ensure clients and staff have opportunities to receive further education related to client’s individualized health needs and medications prescribed
  • Attend to medication reconciliations and updates on MAR (Medication Administration Record) accordingly
  • Serve as an advocate for clients, linking them to needed community services related to medical necessity; assess client’s personal, medical, social, emotional, and environmental situation to plan for linkage and treatment course
  • Assist in transporting clients in the community to medical appointments when necessary
  • Provide individually-based motivational treatment to clients to assist them in their recovery from mental illness
  • Provide client-centered and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders.
  • Potential for occasional phone availability off hours or on weekends to answer questions from staff on the team about specific clients that the nurse has a direct working relationship with to help give direction.
  • Perform other related duties and/or projects that may be assigned to support the agency as a whole

Qualifications:

Must have received or be willing to receive the COVID-19 vaccination and booster by the date of hire to be considered. Proof of vaccination required.

  • Associate degree in Nursing required; Bachelor’s/Master’s preferred
  • Current Registered Nurse license in the State of Illinois.
  • Active Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification required
  • Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
  • Experience working with people with serious mental illness and medical comorbidities preferred
  • Experience in care coordination highly preferred
