Responsibilities:
- Develop, implement, and monitor Community Mapping Plans for assigned residential Individuals to support community membership and to promote individuals having relationships with members of their community
- Develop and implement teaching activities in residences to assist individuals in their personal growth.
- Advocate on behalf of assigned individuals and to be responsible for knowing what is happening in all areas of an individual’s life
- Maintain ongoing communication with families/guardians to support relationships between families/guardians and their family member
- Bachelor’s Degree and two years of related experience
- * Five years of experience may be applied in lieu of educational requirements
- Valid driver’s license & Insured Vehicle
