Title: Collaborative Coordinator/Patient Coordinator

Reports To: Dodge Co. Family Services Collaborative’s Care Council & Steele Co. CMH Governing Board, SCHRC System Administrator, Business Analyst

Employment Status: PT - FT 20-32 hours
SUMMARY

Ensure the effective coordination of the Dodge and Steele County Collaboratives and their projects and programs, including coordination of the Local Collaborative Time Study (LCTS, the Collaboratives’ source of funding). Enhance and enable the organizations and their programs to function in the most effective and productive manner possible.

Provide patient coordinator support to South Central Human Relations Center (SCHRC) clients.

PRIMARY RESPONSIBILITIES

Coordinate the LCTS for Steele, Dodge, and Waseca Collaboratives, including training all new participants and financial contacts (with the help of the Fiscal Reporting and Payment Agent). Ensure random moments and cost reports are received by DHS in a timely manner. Follow up with any concerns that arise.

Partner with FRAPA to complete annual reports.

Distribute Title IVE Candidacy lists to appropriate entities for LCTS.

Schedule and attend board meetings, create agendas, and distribute meeting minutes. Update the boards on LCTS news/developments.

Communicate with DHS on LCTS and Collaborative issues as needed.

Dodge Collaborative

Coordinate the Preschool Scholarship Program and communicate with other grantees providing services to which the Collaborative contributes funds (Semcac, Big Brothers Big Sisters of SE MN, Family Service Rochester)

Coordinate all aspects of the annual Parent Resource Fair.

Coordinate parent education classes each school year.

Partner with Public Health to conduct the Alcohol/Tobacco Decisions (Students Teaching About Refusal Skills) program to the Hayfield, Kasson-Mantorville, and Triton school districts each school year.

Steele Collaborative

Process Wraparound Fund requests and payment, as well as Prevention and Training requests.

Communicate with entities that are eligible to receive prevention and training funds, oversee funding of mental health providers and any other budgeted expenses.

Both Collaboratives

Collaborate with Mn Prairie County Alliance on funding of the Education Stability position. Communicate entities that are eligible to receive prevention and training funds, oversee funding of mental health providers and other budgeted expenses. Coordinate contracts, update budgets, communicate opportunities for funding, collaborate with area agencies and schools to monitor the needs of the county youth and families and work to meet those needs. Responsible for the revision, dissemination, and collection of all program outcome evaluations

Participate in committees that pertain to our goals.

Patient Coordinator

Serve clients by scheduling appointments, maintaining records.

Complete Comprehensive Evaluation paperwork with new clients on their first visit.

SECONDARY RESPONSIBILITIES

Perform miscellaneous projects and complete various tasks as requested by management.

JOB SPECIFICATIONS

Education, Experience and Credentials

Bachelor’s degree in the human services field. Prefer at least one year experience working in a human services position. Prefer experience in working with boards and bringing multiple entities to the table to work towards common goals.

Ability to organize and prioritize one’s own work

Ability to communicate accurately and effectively through oral and written communication

Fluency in English and another language is a preferred qualification but not required.

Ability to work with a board of directors to accomplish Collaborative goals

Ability to use human relation skills to positively interact with and to work constructively with community members

PHYSICAL REQUIREMENTS

In varying degrees and amounts seeing, hearing, talking, walking, standing, sitting, lifting, stooping, crouching, kneeling, reaching, pushing, pulling, and repetitive motions are needed.
