Job description
Essential Duties And Responsibilities:
  • Perform housekeeping, laundry, and other tasks. (See attached task list).
  • CNAI only – (See attached task list).
  • CNAI + 4 - (See attached task list).
  • CNA - I and CNA ll – (See attached task list).
  • Communicate changes, concerns, and potentially problematic patient/caregiver issues to the interdisciplinary team members.
  • Attend interdisciplinary meetings and work as a team member in addressing patient/caregiver needs.
  • Provide quality care to the terminally ill and offer support to the patient and their caregivers as needed.
  • Reinforce safety measures with patients and families.
  • Provide appropriate support at the time of death.
  • Document care provided timely and accurately, direct and indirect patient care time, and administrative time supportive of the organization’s financial and statistical reporting needs by 11:00 a.m. the following business day.
  • Develop a learning plan and review it with your leader.
  • Must complete a minimum of 12 hours of in-service training each 12-month period.
  • Demonstrates familiarity with policies of Carolina Caring; understands and adheres to rules, regulations, and standards of regulatory bodies, QAPI program, and applicable accrediting agencies.
  • Complies in a timely manner with all mandatory in-service requirements and competencies.
  • Accepts other assignments as appropriate.

Job Requirements.
Education:
  • Certified as a Nursing Assistant. NA I must have a current listing as NA I with DFS.
  • NA II must have a current listing as NA I with DFS and a current listing as NA II with the North Carolina Board of Nursing.
  • Must maintain CPR certification.

Experience:
  • One to three years of clinical experience is preferred.
  • Hospice or home health care experience preferred.

Skills/Abilities:
  • Ability to make meaningful observations and complete required documentation.
  • Ability to establish and maintain good relationships with patients and their caregivers.
  • Understands and supports the hospice philosophy of care.
  • Ability to work with an interdisciplinary team.
  • Competency shall be demonstrated by passing the competency and skills examination by a qualified licensed nurse.
  • Proficient computer skills with MS Office products.
  • Excellent customer service and communication skills.
