The Hills Post-Acute is a Santa Ana’s Premier Location for Short-Term Rehabilitation and Long-Term Care. We are a modern, state-of-the-art skilled nursing facility, conveniently located on Old Tustin Avenue near 17th Street in Santa Ana. We feature 174 beds and beautiful spacious rooms in either private or semi-private settings. Our guests enjoy well-appointed accommodations, a diverse activities program, nutritious satisfying menus, housekeeping and laundry services, and beautifully landscaped grounds. Our in-house therapy team employs a variety of state-of-the-art and (good old-fashioned) hands-on therapeutic approaches. We offer both short and long-term rehabilitation that is tailored individually for each of our guests.

We are looking for a caring and professional individual who is passionate about helping people recover from disabilities. You will be required to assist with activities of daily living in addition to assisting in rehabilitation techniques such as physical therapy, occupational therapy, and speech therapy. The successful candidate will be required to work extended hours and work independently as well as with a team of professionals.

Responsibilities:

• Perform basic nursing tasks as directed by RN to ensure that patient's health is not further compromised.
• Take vital signs.
• Monitor patient condition.
• Communicate changes in patient's physical and mental state to doctor or registered nurse.
• Perform hygiene care for patients.
• Assist in moving and repositioning of patients under direction of nurse or physical therapist.
• Help patients with bathing and grooming under direction of a nurse or physical therapist.
• Assist with activities of daily living such as meal preparation and bathing; help patients make beds and clean rooms.
• Provide transportation for patients to appointments, recreation, etc.

Job Types: Full-time, Part-time

Pay: From $18.50 per hour
