I) Job Purpose

The Youth Clinical Therapist Associate is a direct care position providing individualized, trauma-informed, culturally sensitive, and linguistically competent therapeutic services for youth and their caregivers.

II) Essential Functions

A) General requirements of Clinical Therapist Associate

1) Make contact with the youth and family to encourage and maintain involvement at a frequency based on the strengths, needs, abilities, and preferences of the individual as identified on the Recovery Plan and agreed upon by the youth, family, and multidisciplinary team

2) Deliver evidenced-based treatment of mental, emotional, behavioral, and substance use disorders and/or conditions. This includes (but not limited to) mental illness, emotional disturbances, substance use, and co-occurring disorders

3) Provide direct care services to designated youth and families face-to-face

a) Face-to-face contact with youth and family no less than 2 times per month

b) Contact in residential (home) setting not less than 2 times per month

c) Services are provided in locations that meet the needs and preferences of the individuals. Such locations may include residences, schools, correctional settings, shelters, day habilitation centers, hospitals, medical facilities, and/or other service sites

4) Participate in treatment team meetings and case staffings

5) Maintain current health records, which includes (but is not limited to):

a) Registration

b) Fee Assessment, & Assignment of Benefits

c) Maintenance of documentation confidentiality satisfying requirements of HIPAA and other federal and state privacy laws, including privacy rights

d) Consent(s) to comply with privacy and confidentiality requirements

e) Assessments, which may include (but not limited to):

i) Agency-required & waiver-required assessments

ii) Child and Adolescent Needs and Strengths (CANS)

iii) Patient Health Questionnaire (PHQ-9)

iv) Columbia-Suicide Severity Rating Scale (C-SSRS)

v) NICHQ Vanderbilt Assessment Scales

vi) Adverse Childhood Experiences (ACEs)

vii) Multidisciplinary Assessment

f) Recovery Planning

g) Treatment preferences including advanced directives

h) Documentation of all direct, indirect, and attempted contacts with individuals involved in behavioral health services (utilizing collaborative documentation when individual is present and as appropriate)

i) Communications and records provided by outside providers

j) Safety Planning

k) Transition and Discharge Planning

B) Provide clinical services with youth and families

1) Provide emphasis on building positive relationships with individuals

2) Evaluation of the environment

3) Empower individuals to manage their own behavior

4) Utilize therapeutic modalities based on the strengths, needs, abilities, and preferences of the individual

C) Develop Comprehensive Recovery Plan

1) Utilizing the completed assessment and treatment team input, facilitate the development of a person- and family-centered, recovery plan, which includes:

a) Identifying strengths, values, and needs (including cultural and developmental needs)

b) Identifying expressed preferences and outcomes, supports, interdisciplinary treatment team involvement, and crisis planning

c) Review and update assessment and recovery plan at least every 90 days

D) Coordination of care and services

1) Assess, provide referrals, and follow-up on all case management needs including (but not limited to):

a) Financial

b) Legal

c) Social

d) Educational

e) Medical

2) Coordinate and monitor access to programs and/or services, which may include (but not limited to):

a) Skills Training

b) Individual, Family, and/or Group Therapy

c) Support Services

d) Youth Center Specialized Programs

e) Psychiatric & Pharmacological Services

f) Substance Use Services

g) Crisis Prevention, Management, and Postvention Services

E) Coordination with outside providers

1) Track transitions of care between treatment settings including outpatient, emergency, inpatient, and residential services and programs

2) Maintain sufficient communication with assigned individuals and families to be aware of admissions and discharges to alternate care settings

3) Assist in coordinating admission when appropriate, maintain contact throughout the stay, assist in planning for discharge and transition back to outpatient care, and follow up after discharge

4) For individuals admitted to care due to potential risk of harm to self or others, follow-up within 24 hours of discharge to provide postvention crisis services

5) Develop a safety plan for assigned individuals in conjunction with the individual and identified supports

6) Safety plans will be developed within 14 calendar days of admission and will be reviewed and updated as circumstances change, but not less than once every 90 days

7) Maintain communication with care providers and facilitate transfer of medical records between providers as needed

F) Assist with intakes for new individuals in services

1) Complete CANS for new youth in services

2) Complete the BH Youth Initial Assessment for new youth in services

3) Develop initial Person-Centered Recovery Plan with youth

4) Work with a Licensed therapist to complete intake process

G) Maintain compliance with all requirements for supervision in pursuance of full licensure including, but not limited to:

1) Meet at least minimum required hours per month with a board-approved clinical supervisor

2) Maintaining up-to-date records of work hours and clinical supervision hours as required by appropriate state board

H) Persistently maintain a culturally competent and culturally sensitive environment in work locations for all youth and caregiver engagement at all times. Staff member is expected to behave in a culturally sensitive manner in all professional environments, regardless of location

I) Work as a member of Child and Family Services Team

1) Attend all scheduled staff meetings

2) Provide input and engage in problem solving with other team members

3) Serve on provider services/agency workgroups as agreed upon by Director

4) Maintain positive, professional attitude in interactions with other team members 100% of the time

5) Adopt trauma-informed practices into every level of operations and encounters through active involvement in the MHMR Tarrant Trauma-Informed Care transformation

J) Complete and maintain all required training

1) Complete all agency-required training within first 60 days of hire

2) Complete all required MHMR Tarrant training and maintain current training status

3) Complete all required program-specific and role-specific training and maintain current training status

4) Complete training updates as specified under agency procedures and maintains current status

5) Obtain prior authorization from Director for special training at least two weeks in advance

IV) Internal & External Customer Service

This position requires extensive internal and external contacts with individuals served, family members, significant others, inter- and intra-agency staff, service providers, and practitioners. The employee will accomplish this with exceptional written and verbal skills. Contacts may include, but not limited to, answering phone calls and emails within one business day and conducting self professionally in all interactions.

V) Travel

This position may require significant travel within Tarrant County. A company vehicle may be available, or you may be expected to use your personal vehicle. Staff is responsible for maintaining accurate records and submitting these on time in order to receive compensation allowed as determined by division and agency policy. Staff is responsible to pay, in full, any traffic tickets and fines for traffic violations while working. Tickets received by agency due to a staff’s traffic violation, while driving a company vehicle, are deducted from staff paycheck.

A) This position requires frequent travel to agency program sites.

B) This position requires frequent travel to community and residential sites.

C) This position requires occasional transport of agency employees and individuals served.

D) This position requires occasional overnight state-wide travel.

E) This position requires travel in personal and agency vehicles.

F) This position requires completion and regular updates of agency approved Defensive Driving training

Agency Requirements

All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.

This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.

Assigned work hours may change as the needs of the agency change.

The Functional Title of this position may change as the needs of the agency change.

All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.

Prior approval from supervisor is required for all Paid Time Off (PTO) and Overtime.

MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.

Employment is at will, as well as agency needs may change.

Agency dress code is to be followed at all times.

Regular and predictable work-site attendance, and punctuality is an essential function of this position.

Physical on-site presence, including regular attendance and punctuality, is an essential function of this position. Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
