Schedule: Mon-Fri 8:00am-4:00pm (no nights, weekends, or on-call!)

Salary: $70,300 - $74,000 / year (depending on licensure)

Sign-on Bonus:$4000; $1333 upon completion of 30 days, $1333 upon completion of 6 months, $1334 upon completion of 12 months

Qualifications:

  • Master's Degree in a clinical counseling license-eligible field required.
  • Required Credentials/Licenses: License to practice as "Licensed Practitioner of the Healing Arts" in MA required. License to practice independently (LICSW, LMHC) preferred.
  • CPR and First Aid required within two weeks of hire

As a Program Coordinator you will collaborate with the Program Manager to provide programmatic oversight and direct supervision for an identified mini team. The Program Coordinator provides care coordination and clinical care management for One Care enrollees with complex healthcare and behavioral health needs. The Program Coordinator collaborates with Vinfen's ACCS teams and the One Care Plan's clinical staff to promote integrated care, ensure quality and continuity of care, while minimizing duplicative efforts. The Program Coordinator will be at the helm of organizing and coordinating resources and services in response to the individual healthcare needs across multiple settings.

Program Coordinators have a range of duties including:

  • Collaborate with the Program Manager to implement care team policies, procedures, and work flows to ensure all One Care program functions and contractual requirements are efficiently met.
  • Provide direct supervision to members of the identified mini team.
  • Implement quality improvements as driven by program data.
  • Serve as a liaison to any or all of the following: the Department of Mental Health (DMH) Site/Center and Area staff; Behavioral Health Care providers; local Emergency teams; Inpatient and Partial Hospitalization staff; Guardian or family members, and other involved providers, as necessary or requested.
  • Promote the utilization of evidence based and promising practices that optimize care integration and effective communication across systems of care and that align with rehabilitation, recovery, and wellness principles.
  • Outreach to and engagement of clients enrolled in One Care
