What Will you be Tasked With?
  • Assists with ensuring office is compliant with all applicable state, federal, local, Medicare and Medi-cal regulations.
  • Provides leadership and supervisions to all clinical staff supervised and conducts and coordinates case conferences, in-service programs and team meetings as requested.
  • Takes referrals (as needed) and makes initial assessment visits, or ensures that assessment visits are made and makes intermittent and on-site supervisory home visits when directed or necessary and organizes family resources for effective patient care.
  • Demonstrates leadership in promoting and maintaining standards for quality care and supervises health care practices to assure that the standards of care are maintained.
  • Ensures the completion of competency and performance evaluations of staff and documentation of same as required.
  • Responsible for the effective, efficient patient care assignment, scheduling and supervision of staff and on-call coverage and maintains records of case assignments and management forms as requested.
  • Organizes and directs patient care conferences and works effectively with Health Care Providers and other members of the home health team to promote coordination of patient services, positive patient outcomes and effective documentation of same, as requested.
  • Reviews medical records routinely and conducts clinical record reviews to supervise the quality and timeliness of care and documentation. Investigates adverse events to identify opportunities to improve care and follow-ups as requested.
  • Serves on professional Advisory Committee and assists with quality/performance improvement monitory activities, as requested.
  • Supervises staff effectively and completes performance actions as indicated as documents appropriately.
  • Informs the DPCS/Administrator of any unusual occurrence, accidents or adverse effects of medical devices and documents appropriately and assists with follow-up as necessary.
  • Adheres to and supervises compliance with policies.
  • Other duties as assigned by the Director of Patient Care Services.


Environmental Conditions and Physical Requirements
  • Driving or riding in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

Minimum Requirements:
  • Graduation from an accredited school of nursing and current RN licensure in the state of practice.
  • At least 2 years clinical nursing experience within the last 5 years. Home health supervisory experience preferred.
  • Excellent interpersonal, verbal and written communication skills and ability to recognize the needs and concerns of people and maintain constructive working relationships with them.
  • Must have a vehicle, driver’s license and appropriate automobile insurance coverage.
  • Ability to make home visits and provide skilled nursing assessments and care in the home environment.
