Job Scope : The Clinical Diversion Assessment Specialist provides comprehensive clinical mental health and level of care assessments to adults from local hospitals referred to the Front Door Team. Assessment includes review of records, family consult, in-person assessment, outreach, referrals, and treatment recommendations to assist adults avoid nursing facility admission and maintain in a lesser level of care and remain in community-based settings after experiencing a mental health crisis. Treatment services are delivered at flexible times that meet clients’ needs and in settings that offer the best potential for positive outcomes, including in hospital and nursing home centers, in-home, and in community-based settings. Coordinate with KYC team members, with network of hospitals, PAS agencies, and other local community mental health services.
Proof of COVID-19 vaccination required. Accommodation for medical and religious exemption will be considered. Weekly testing required for approved v accination e xemptions.
Clinical Duties
  • Complete comprehensive strengths-based, client-centered, and trauma-informed clinical mental health assessments to adults at local hospital and specialized mental health nursing facility (SMHRF) settings
  • As a member of assessment team, respond to referrals by reaching out to hospitals and completing assessment within 24 hours of referral to meet grant requirements
  • Provide on-site intake at various community access points, by providing mental health assessment, treatment recommendations, and discharge planning activities in hospitals, at nursing homes, in the community, and other locations as assigned.
  • Maintain a high level of personal and team documentation compliance including timely review and signing of team members’ integrated mental health assessments and treatment plans (IM-CANS).
  • Cross-train to provide outreach, case management, linkage and referral services to individuals discharging from hospital settings for up to 180 days to assist clients to continue to avoid institutional levels of care.
  • Cross-train to conduct phone check-ins and/or in person meetings to ensure client contact and provide coverage
  • Perform and participate in crisis intervention when necessary.
  • Routinely consult with other Behavioral Health team members including employment services, psychosocial rehabilitation, SUPR/MISA, drop-in staff, and psychiatry to address specific client related clinical issues and treatment recommendations
Administrative
  • Complete all clinical documentation and required reports in an accurate and timely manner; including referrals, desk reviews, IM-CANS, and other assessment tools
  • Interpret and communicate, at the service delivery level, Center policies and procedures
  • Meet or exceed productivity target estimated at 50-60%
  • Participate in orientations, all staff trainings, and training and development seminars
  • Submit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Center’s policies and procedures
  • Coordinate with program support regarding referral status and state communication
Education and Experience
  • Preferred Masters Degree from an accredited College or University in Social Work, Psychology, Counseling or closely related discipline; Bachelors Degree with significant mental health assessment experience will be considered
  • Experience working with adults with serious mental illness
  • Experience completing comprehensive mental health assessments
  • Preferred experience working in non-traditional treatment environments i.e.in-home, residential, hospital, community settings
  • Preferred experience working with individuals with chronic homelessness and/or substance use difficulties
  • Applicant must have a valid IL Driver’s License, proof of insurance, and daily access to a reliable well maintained vehicle
  • Computer and smartphone proficiency, working knowledge of MS Word, Excel, Outlook, PowerPoint, Adobe PDF, team communication platforms (ex Microsoft Teams), Internet skills, and smartphone applications and messaging
