This is a remote position with expected travel of up to 30% throughout the Jefferson Region for member facing visits. Schedule is Monday-Friday, standard business hours, 8:00-5:00pm.

Candidates must reside within the Northern Blue Grass Region. It includes Boone, Bourbon, Campbell, Carroll Gallatin, Grant, Harrison, Kenton, Nicholas, Owen and Pendleton counties of KY.


The Clinical Case Manager - BH is assigned as primary liaison to all acute psychiatric and psychiatric residential treatment facilities in the Northern Blue Grass Region of KY.

Goals for Clinical Case Manager - BH include to reduce readmissions, reduction of polypharmacy utilization, increased placement stability, assure highest quality of care for members, assist in treatment collaboration, advocacy, reduce length of stays as clinically appropriate.

Pay Range
The typical pay range for this role is:
Minimum: 58,760
Maximum: 125,840

Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.

Required Qualifications

• 2+ years experience with multisystem involving youth and DCBS required.
• 2+ years of experience in behavioral health services required.
• Active and unrestricted Clinical Behavioral Health License in the state of KY (LCSW, LMFT, LPCC).
• Willing and able to travel 30% of their time using their own vehicle to meet members face to face within the assigned Northern Blue Grass Region.
