POSITION SUMMARY

This position is responsible for providing case management services to Willimantic and Danielson Behavioral Health Center clients to assist with ongoing, referral and discharge treatment needs. This position will act as a point person with other agency outreach and case management programs.

Hourly rate depending on experience $19.23 - $ 21.63


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Complete state reporting Functions (DDaP, Level of Care changes, discharges, etc.).
  • Facilitate level of care changes and site transfers.
  • Complete insurance authorizations.
  • Conduct concurrent reviews.
  • Act as main contact for communication with collateral contacts.
  • Draft and submit request of information documentation.
  • Assist with client discharge to include linkage to mental health and substance use treatment services, HLOC referral and completion of discharge summaries.
  • Run reports as needed (e.g., client engagement reports, discharge, etc.).
  • Assist engaged clients who have been referred by their clinician with general CM ( if they don’t qualify for contracted CM).
  • Provide education and coaching, particularly in available resources within the local recovery community.
  • Monitor adherence to program benchmarks and requirements, reporting as necessary.
  • Perform other duties as assigned.

MINIMUM REQUIREMENTS

  • Bachelors degree in related field. Experience may be considered in lieu of degree.
  • Minimum of 1 years’ experience in related field.
  • Valid driver’s license.
  • Prior knowledge of mental health and substance use strongly preferred.
  • Lived experience with mental health and/or substance use disorders strongly preferred.
  • Ability to learn new systems and programs quickly.
  • Excellent verbal and written communication skills.
  • Excellent customer service skills.
  • Ability to develop clear and professional documentation.

  • Willingness and ability to establish and maintain healthy working relationships.
