Accucare Nursing and Home Care is actively seeking qualified applicants for the position of Children's Health Home Care Manager.
This position involves providing intake, service coordination, and ongoing care management to medically fragile individuals within the Children's Health Home program living in the following counties: (Orange, Rockland, Westchester, Sullivan, Dutchess, Ulster, Putnam, and Bronx.)
Specific Duties
- Provide youth-guided and family driven care management to services to assigned caseload.
- Document all case activity.
- Complete comprehensive assessments, care plans, case reviews and other detailed assessments as required.
- Provide services for each family which include face to face visits per month and core services that correlate with goals as outlined in care plans.
- Provide referrals and coordination of services related to all aspects of the youth's social, physical and mental health.
- Assist families in the acquisition and maintenance of public benefits.
- Participate in staff meetings, ongoing training and other professional development on a monthly basis.
Qualifications
BA with two years experience or relevant experience in the field. Health Home experience a plus.- Early Intervention Ongoing Service Coordination experience a plus.
- Excellent writing, verbal and phone skills.
- Ability to work in self-directed manner.
