The Mobile Medication Service is a voluntary service provided for adult individuals with serious mental illness who experience difficulty making progress toward recovery. The Mobile Medication Service is designed to create a continuum of care related to medication administration, as well as assessing, monitoring and education that span the system of care for that individual.

The Peer Support’s function on the team is to uniquely engage the individuals, assess their needs/barriers to care and to reinforce what has been learned and the generalization and application of that knowledge. The Peer Support works to educate, support, empower and engage with individual in service to promote quality, holistic care to ensure healthy outcomes, wellness attainment, and ongoing tenure in the community setting. The scope of responsibility of the team includes counseling, crisis intervention, promotion of self-care skills, monitoring of medical, physical, and mental health status/treatments, and coordination of services.

Minimum Qualifications

Education: High School Diploma or GED is required. Certified Peer Specialist.

Experience: Experience as a consumer in the mental health system and familiar with recovery principles and practices.

Licenses, Certifications & Clearances

  • Act 33/34 (PA State Criminal and Child Abuse) Clearances
  • FBI Clearance (prepared by PDE)
  • Valid PA driver’s license
  • Certified Peer Specialist

Essential Duties & Responsibilities

  • Support and educate individuals in service.
  • Develop and promote health and wellness goals with individuals in service.
  • Capacity to maintain a helping role and to intervene appropriately to meet service goals.
  • Complete documentation of service provision as per program requirements.

  • Ability to be sensitive to the service population’s cultural and socioeconomic characteristics.
  • Commitment to empowering others to solve their own problems.
  • Commitment to valuing a nurturing family as the ideal environment.
  • Conviction about the capacity of people to grow and change.
  • Ability to establish a respectful relationship with individuals served to help them gain skills and confidence.
  • Knowledge of mental health/substance use and the related services.
  • Ability to work collaboratively with individuals, family members, care givers and other service providers or professionals.
  • Ability to exercise sound judgment and to communicate effectively orally and in writing.
  • Ability to be punctual and committed to the role.
  • Ability to exercise flexibility and creativity in working with others.
  • Ability to set appropriate limits with individuals in service.
  • Supports individuals as active participants in treatment planning.
  • Aids to decrease the utilization of crisis and inpatient services due to medication management issues.
  • Assists to ensure success in a least restrictive living arrangement and level of treatment care
