What Will you be Tasked With?
  • Communicates with the Health Care Provider and other members of the health care team for the delivery and coordination of quality occupational therapy services.
  • Demonstrates thorough knowledge of the principles and practices of occupational therapy assistant and provides occupational therapy assistance services in accordance with the Health Care Provider’s plan of care.
  • Participates in the ongoing evaluation of patient's functional status (muscle function, endurance, visual coordination, written and verbal skills self care ability work capacity) as delegated by the Occupational Therapist.
  • Participates in the ongoing evaluation of the home environment for hazards or barriers to a more independent living as delegated by the Occupational Therapist.
  • Follows treatment program and goals for improved patient function as established by the Occupational Therapist.
  • Participates in teaching new skills or retraining patients in once familiar daily activities that have been lost due to injury or illness as directed in the plan of care.
  • Performs occupational therapy assistant procedures, measurements, evaluations and diagnostic tests on patients in the home for whom they have been medically prescribed to aid in establishing or revising specifics of treatment program and communicates results with Occupational Therapist and clinical care team.
  • Observes, records and reports to health care delivery team, the patient’s treatment, response and progress and promptly reports significant changes and documents accordingly.
  • Communicates regularly with designated Clinical Manager and other health care team members to coordinate services, evaluate patient’s progress and update the plan of care.
  • Helps patients and families to accept and adjust positively to physical, mental, and social limitations and refers patients to social services, when necessary.   • Plans for coordinated service for patients under care by consulting and working with other health care team members effectively and communicating schedules, as assigned.
  • Completes clear concise and accurate clinical notes, which document skilled services appropriately, and submits all requested documentations on a timely basis (24hrs).
  • Participates in orientation, case conferences, in-services and staff meetings as requested.
  • Assists with orientation as directed by the Occupational Therapist.
  • Uses proper infection prevention control procedures, universal precautions and safety measures.
  • Serves as a member of the Professional Advisory Committee and participates in quality performance/improvement activities as requested.
  • Adheres to agency’s policies and procedures.
  • Provides and documents services in accordance with applicable State, Federal, Medicare, Medi-cal regulations and standards and third party payer guidelines.
  • Instructs patient/CG in the use of alternate methods and devices for upgrading independence in activities of daily living.
  • Designs, fabricates, and adapts self-help orthotic devices; including ordering and issuing equipment as needed.
  • Helps individual and families to accept and adjust positively to physical, mental and social limitations and refers patients to social service as necessary.
  • Teaches basic principles of good posture, body mechanics and work simplification techniques to patients.


Environmental Conditions and Physical Requirements
  • Driving or riding in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

Our San Diego Home Health Branch Treats Patients in the Following Cities:

San Diego/Chula Vista

Carlsbad/San Marcos

Temecula/Murietta

Riverside/Corona

Minimum Requirements:
  • Bachelor’s Degree and graduation from an occupational therapy institution that is jointly accredited by the Committee on Allied Health Education and Accreditation of the AMA and the American Occupational Therapy Association.
  • Licensed as an Occupational Therapist in the state of practice.
  • Work experience of at least one (1) year under a qualified Occupational Therapist, home health experience preferable.
  • Must have a vehicle, current driver’s license and appropriate automobile insurance.
  • Ability to make home visits and provide physical therapy evaluations and treatments in the home environment complete documentation effectively and submit in a timely manner.
Preferred Requirements:
  • Home health experience
  • Home Care Home Base documentation experience
