The CBHC Case Manager is an integral part of the team, responsible for assisting in development of treatment plans, completion of required assessments and ensuring that all service notes related to persons served and collaterals are accurate and up to date. You will also assist the team in providing an on-going assessment of symptoms and responses to staff interventions while guaranteeing treatment goals are culturally competent, strength based, realistic, attainable and promote recovery.

Responsibilities:

  • Provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragement.
  • Provide interventions in the office, virtually, in the community and in client’s residences based on client preference, client need, and at the clinical direction of the Team.
  • Complete service notes related to all contacts with individuals and collaterals on the day of contact. Participate in daily Team Huddles and Team Meetings including completing documentation of all client encounters prior to the Team Huddle meeting and providing oral reports on each client.
  • Complete NOMS assessments as assigned, aspects of clinical assessments, and other documentation and required to meet CBHC/Eliot standards.
  • Assist the team and individuals in providing on-going assessment of symptoms and responses to staff interventions.
  • Participate in the development of treatment plans for each individual in which you are a member of their treatment team. Ensure that treatment goals are culturally competent, strength based, realistic, attainable and promote recovery.
  • Ensure that each individual is actively involved in their treatment planning process and their preferences are embraced.
  • Ensure that documentation is written is measurable, observable, objective and utilizes clinical terms.
  • Provide interventions to individuals as stated by their current treatment plan or in response to the daily needs of individuals per supervisory direction and/or through the Team Huddle process.

Qualifications:

  • Must have bachelor’s degree or master’s degree in psychology, social work or related field and three years experience.
  • Must have a strong commitment to recovery oriented services.
  • Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses, kids, families and respect for their preferences for treatment.
  • Must have ability to communicate clearly and effectively with individuals in rehabilitation and treatment.

Salary:

  • Bachelor level: $48,000
  • Master level: $52,000

Various schedules available.

Bilingual candidates preferred and eligible to receive a 6% differential if fluent.

Bonus Breakdown:

  • $1500 after 1 month of employment
  • $1500 after 6 months of employment
