Community Support Facilitator for Supported Living Services

Strategies To Empower People (S.T.E.P.), a Supported Living Services agency that supports adults with developmental and/or intellectual disabilities, is currently seeking a qualified individual to fulfill our full-time Community Support Facilitator position currently available.

We are looking for a Facilitator (Case Supervisor) to support our clients who reside in the Sacramento County area. This position will be responsible for traveling between Sacramento, Citrus Heights, Rancho Cordova, and more areas.

MUST meet age and driving requirements: 25 years of age or older and a good driving record. Willing to participate in our DMV Pull Program.

Pay and Benefits:
$62,400 annually. Mileage reimbursement at 68.5 cents per mile.

*This is NOT a Monday to Friday, 9a to 5p position. You will need to be able to work a flexible work schedule Monday thru Friday with a rotating on-call weekend schedule. This is not an office job.*

YOU WILL BE REQUIRED TO HAVE EXCELLENT TIME MANAGEMENT SKILLS IN ORDER TO BE SUCCESSFUL IN THIS POSITION.

This position requires you to provide instruction and support to adults with intellectual/developmental disabilities while following the Supported Living Services principles and S.T.E.P's vision and mission statements. This position will involve working with the people we support, whom we refer to as our "employers," and actively working with them toward meeting their ISP (Individual Service Plan) goals as well as personal goals in their homes and in the community. You will promote and teach self-advocacy, personal, domestic, medical, financial, social/recreational, communication, and behavior management skills to our employers to encourage their independence.

This individual will be expected to promote and teach the above attributes to adult individuals with developmental disabilities who may have significant medical and/or behavioral issues. This person will be expected to complete all required paperwork accurately and submit by scheduled due dates. Required paperwork includes, but is not limited to, client case notes, progress reports, employee time worked/time cards, client assessments, supervisor documentation, Special Incident Reports, financial and medical logs, Adult Protective Services reports, and much more. This person will also be responsible for supervising employees in the employer's home (can be as much as 30 staff) and is responsible for arranging and securing the staffing required in these homes. Generally, a Case Supervisor/Facilitator is responsible for a caseload of approximately 5 to 6 employers and the staffing associated with them.

You will be expected to maintain a 75/25 ratio for your time; meaning 75% of your time will be with your employers, and 25% of your time will be administrative time (reports, documentation, financial/medical auditing, etc.). There will be many meetings, many deadline driven reports and forms on a monthly, semi-monthly, and quarterly basis. At times and when needed, you will be asked to perform the same duties (personal care/hygiene, housecleaning, cooking, etc.) as the employees that you will supervise. You will receive all necessary trainings so that you are capable of performing all the duties required of this position.

There is a need to be able to successfully work independently with minimal supervision AND to successfully work as part of a team.

-Be a High School graduate or possess a GED or equivalent.

-REQUIRED: Possess two or more years of experience working with individuals with developmental and/or intellectual disabilities and a good understanding of the developmental disabilities field. Experience working with individuals who are highly medically or behaviorally challenged is preferred.

-REQUIRED: One or more years of previous supervisory experience is required (supervising employees) and budgetary/finance experience.

-PREFERRED: Experience working in Supported Living Services or similar.

-REQUIRED: Possess a valid CA driver license, safe, reliable, registered and currently insured vehicle.

-MUST meet age and driving requirements: 25 years of age or older and a good driving record. Willing to participate in our DMV Pull Program.

-Must be computer literate and be able to quickly and competently learn and use new computer programs and methods. Must be able to compile good reports using a variety of computer programs.

-Possess current CPR/1st Aid certification; if hired, must maintain throughout employment with agency.

-Be cleared through a criminal background check and successfully pass post-offer, pre-employment drug screen.

-Be able to read/write in the English language in order to perform essential duties and possess basic mathematical skills.

-Be punctual, flexible, reliable, adaptable, creative, patient, self-confident, and trustworthy.
