Volunteers of America Northern New England is seeking a motivated individual to be part of our Veterans Transitional Housing Team.

Position Overview: As part of the Veterans Administrations initiative to assist homeless Veterans with housing retention in Central and Southern Maine, the Veteran Support Service Case Manager is responsible for coordinating casework services to Veterans with a focus on maintaining stable permanent housing. This job is a Full-time, 40-hour/week, M-F position. Wage is $22.50/hour.

Position Type: Hybrid Remote with Travel to Client Homes Required.

PLEASE NOTE: While a portion of the scope of this position is remote work, this position also requires travel to client homes in Maine. To apply, you must be a Maine resident.

Essential Job Functions:

1. Conduct in-home and face-to-face visits as well as telephone outreach to Veterans transitioning to permanent housing and manage client caseload assigned according to the program contract.

2. Model appropriate behavior and maintain appropriate boundaries.

3. Monitor Veteran stability using client acuity assessments as and when required.

4. Create and review Individual Service Support Plans in conjunction with the client and document Veterans’ progress.

5. Identify significant family and community supports needed and develop a comprehensive, non- redundant case plan that encompasses a wraparound system to include multiple service providers. Modify the plan as client circumstances and needs dictate; develop and maintain comprehensive resource files of information on organizations that provide human services in the community.

6. Ensure provision of client services according to the case plan, including services such as drug and alcohol counseling/referrals and workshops, drug screening and monitoring, job development, and referral to community resources. Document those services in each client’s case file.

7. Instruct clients in basic financial skills, including budgeting, savings, fiscal responsibility, and mechanisms to develop and build credit, assist clients in dealing with their total health concerns and the effective utilization of established health agencies, and provide critical intervention services to clients in the program service area as needed.

8. Coordinate and facilitate cognitive/behavioral and life skills groups for clients, when appropriate.

9. Drive the process for Veteran Referrals and Intake Coordination (which includes Data Management), clearly document all changes in Veterans’ acuity in a timely and accurate manner as well as maintain Veterans’ service files and records.

10. Conduct and collect in-depth new Veteran intake assessments and input new data into AASC Online (web-based information system) for VOANNE clients within 48 hours and develop a tracking system to capture all referrals (accepted and denied).

11. Gather HMIS numbers/data and past case management history when possible.

12. Answer phone calls with requests for information and referrals for clients and other service providers.

13. Develop and maintain comprehensive resource files of information on organizations that provide human services in the community.

14. Maintains positive working relationships with collaborative community agencies and funding sources.

15. Maintain appropriate contact with outside agencies contracted to provide specific services for clients, with Program Manager functioning in an oversight capacity to ensure client’s involvement in the program.

16. Represent VOANNE in working with other community-based agencies to expand service networks.

17. Participate in and contribute to other duties and responsibilities as assigned.

Education, Experience, and Qualifications:

1. A Bachelor’s degree preferred; 4 years of case management or housing navigation experience may be substituted for the educational requirement.

2. Strong skill-set and ability in individual case planning and management, resource development, public relations, verbal and written communications, and group facilitation;

3. Must have strong computer and Microsoft Office Skills including Word processing, recordkeeping, and reporting, email, and the Internet research and use;

4. Ability to maintain all required professional licenses and professional training and also successfully complete other training required by regulation, funding source, or the agency, such as CPR and handling of hazardous materials; and

Must have own (reliable) vehicle and a valid state of Maine driver's license, a minimum of three years' good driving history is required.

Candidates must have a sufficient level of computer skill to effectively use the systems (e.g., Word processing, recordkeeping, and reporting), email, and the Internet as a routine part of daily work activities.

Veteran, or two years’ experience working with Veterans or social services, is preferred.

Travel is required to client homes, work meetings, organizational and team trainings, or other events.
