Job Title: Case Manager

Job Summary: The Veteran Program Case Manager provides support and assistance in carrying out day to day operations of the Veteran Program by performing the following duties:

Responsibilities:

· Prepares incident reports, client file documents, program attendance and data collection relative to services provided as necessary and maintains records of program related activities

· Screens new clients on intake

· Interacts with adult clients and their children

· Assists in supporting the veteran with services and network services available to veterans

· Assists with the maintenance of the program in a clean, sanitary and safe condition

· Documents and maintains proper reporting of incidents and relays them to the Program Manager or Director of Shelter Programs.

· Remains alert to outbreaks of infectious diseases and reports same as directed

· Assists clients with case management activities and provides direction on life skills

· Assists other staff with client and program related duties

· Stays alert to all client activity during the shift hours as assigned

· Fulfills other essential responsibilities and duties as may be assigned and/or required.

· Attends meetings as directed in representation of the agency

· Carries out responsibilities in accordance with the organization's policies and applicable laws. Affects other duties as may be assigned.

· Bi-lingual Spanish speaking preferred.

· Veteran status preferred.

· Performs other related duties as assigned.

Required Skills/Abilities:
· Professional verbal and written communication skills.

· Excellent organizational skills and attention to detail.

· Excellent time management skills with a proven ability to meet deadlines.

· Ability to accomplish goals and produce valuable results with minimal supervision

· Strong analytical and problem-solving skills.

· Excellent professional boundaries

· Maintain and execute confidential information according to HIPAA standards

· Exercise mature judgement, and are highly motivated, self-starting and proactive

Education and Experience:

· High school diploma or equivalent.

· Bachelor’s preferred

· Be able and willing to work flexible hours which may include evenings or weekends

· Have or be able and willing to obtain CPR/First Aid training Provide proof of full COVID-19 vaccination

· Have reliable transportation and:

· A valid driver’s license

· The ability to qualify for Good Samaritan Shelter insurance coverage

Successfully complete the following as a condition of hire:

Tuberculosis Test Background Screening Drug Test

Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
