OVERVIEW

The Refugee Mental Health Case Manager will work to increase refugees’ access to medical and mental health resources in the community. The Mental Health Case Manager will work to build a holistic network of support for mental health services and improve rates of self-sufficiency and integration among refugees.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Increase access to mental and behavioral health resources for refugees;
• Facilitate a coordinated system of mental health care for refugees in the Des Moines area;
• Build the capacity of mental health providers and other service providers to care for refugee mental health needs;
• Implement widespread usage of mental health screening tool developed for refugees;
• Provide direct case management services to refugees in the community;
• Conduct home visits, intakes and assessments of clients referred to the program;
• Coordinate appointments and services, maintain case files and case notes for clients enrolled in program;
• Track client enrollment, outcomes, and interventions, and provide information in regular reports;
• Research and coordinate alternative supportive therapies for refugees, such as art classes, community groups, or music therapy;
• Work together with team members in the Wellness Program and resettlement agency to serve refugees with medical and behavioral health needs;
• Attend meetings of stakeholders in the health community to create new partnerships and share resources;
• Ensure that refugees are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary; and
• Perform other job related duties as assigned by supervisor.

POSITION REQUIREMENTS

• Bachelor’s in social work, psychology, human services, or related discipline;
• Experience in trauma-informed care and strengths-based case management preferred;
• Ability to work as a team member and independently, with high level of self-motivation;
• Ability to work efficiently in a fast-paced environment, troubleshoot, and follow projects through without loss of attention to detail;
• Ability to work independently as well as be a team player;
• Exceptional interpersonal skills, including keen listening ability and relationship building;
• Proficient in Microsoft Office;
• Must have a valid driver's license, insurance, clean driving record and personal transportation; and
• Dedication to the human rights of refugees, immigrants, asylum seekers, and displaced people.

PHYSICAL DEMANDS

• Use of manual dexterity, tactile, visual, and audio acuity;
• Use of repetitive motion, prolonged periods of sitting, and sustained visual and mental applications and demands; and
• Occasional lifting (up to 25 pounds), bending, pulling, and carrying.

TRAINING REQUIREMENTS

• Satisfactory completion of USCRI’s Orientation and Training;
• Complete additional training as identified by supervisor or Human Resources.
