WestCoast Children's Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The Team-Based model of care program named EMPOWR (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

We currently have two Case Manager openings:
1. The Case Manager provides direct support and guidance to youth in the program. The Case Manager works in collaboration with Mental Health Clinicians to provide advocacy, community-based rehabilitation services, referrals and collateral work with parents, social workers, teachers, and other adults in youths' lives. Services may include linking to and coordinating with tutoring & vocational programs, mentoring, housing, health, benefits or other needs of the family. The Case Manager must have strong advocacy skills, demonstrate flexibility, utilize a strengths-based approach, and have demonstrated experience working with youth and their families.
2. The Peer Case Manager provides direct support and guidance to youth in the program. Because of their lived experience, they help create a connection with the youth and offer a unique perspective to the rest of the team. The Peer Case Manager works in collaboration with the EMPOWR team to provide advocacy, community-based rehabilitation services, referrals and collateral work with parents, social workers, teachers, and other adults in youths' lives. Services may include linking to and coordinating with tutoring & vocational programs, mentoring, housing, health, benefits or other needs of the family. The Peer Case Manager must have strong advocacy skills, demonstrate flexibility, utilize a strengths-based approach, and have demonstrated experience working with youth and their families.
How you'll make an impact:

  • Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living.
  • Provides advocacy, targeted case management and community based rehabilitation services to transition-age youth.
  • Assist youth in identifying and utilizing community services and resources to address their needs, including: housing, medical/physical health, education, employment, independent living skills, advocacy, placement stability, and age-appropriate enrichment activities.
  • Provide intensive skill building with youth, with a focus on independent living.
  • Provide risk assessments, crisis intervention, stabilization, and creates safety plans.
  • Assist youth with accessing SSI and/or regional center services, appropriate housing resources including acute and structured residential treatment options as appropriate.
  • Assist youth with obtaining and keeping Medi-Cal.
  • Provide psycho-education about mental health issues and advocacy services for clients.
  • Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.
  • Complies with policies, procedures, documentation and paperwork for EPSDT Medi-Cal billing and meets billing requirements as set by the agency.
  • Provides services in the community, for which travel by car is necessary.
  • Provide stabilization support services to children and TAY in an in-person milieu environment.
  • Demonstrates professional and ethical conduct at all times as a representative of the WestCoast community.

Qualifications:

  • Peer Case Manager: Direct, personal experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.
  • Case Manager AND Peer Case Manager:
  • Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)
  • Experience providing case management and rehabilitation services.
  • At least 2 years experience working with system-involved youth.
  • Demonstrated ability to work with transition age youth and their families who have experienced high levels of trauma and demonstrate high mental health needs.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Experience working either in the community or in a variety of settings: homes, schools, neighborhoods, other agencies, etc.
  • Experience in working with diverse communities.
  • Familiarity with Alameda County resources for families.
  • Ability to complete Medi-Cal documentation and billing.
  • Ability to work with clients who have complex traumatic histories and other attachment difficulties.
  • Proven ability to be organized, manage time well, complete high quality and timely documentation.
  • Clean CA DMV record and current car insurance and car; travel is required.

Competencies (skills, abilities, and knowledge):

  • Demonstrated experience working with children and families who may be involved in the child welfare system.
  • Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
  • Cultural humility - knowledge of issues of race, class and ethnicity.
  • Efficiency and time management - ability to be well-organized and complete timely documentation.
  • Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
  • Ability to work with clients who have complex traumatic histories and other attachment difficulties.

Position details and compensation:
Salary starting at $65,000 per year depending on degree/experience, plus a 10% bilingual language differential, if applicable. Includes competitive benefits package, 403(b) plan, and performance-based compensation.
