JOB DESCRIPTION

Administrative

  • Ensure adequate ROIs for coordination of care and check ROIs before any outside contact
  • Determine financially responsible party (FRP)
  • Determine legal issues (guardianship, DCS, probation/parole, etc.) and other factors influencing treatment
  • Determine additional case management needs (EAP, FMLA, return to work, etc.) and coordinate with appropriate staff

Collection of Information

  • Collect and document input from client on continuing care needs
  • Collect input from primary therapist and provider for continuing care recommendations
  • Coordinate with UR/BO teams to estimate and adjust length of stay
  • Contact FRP, families, referent, TPSs on admission for coordination of care and document
  • Maintain brief weekly contact with FRP, family, referent, and TPS (as requested) and document
  • Update discharge/stepdown tracker daily

Connection to Resources

  • Provide recommendations to residents for continuing care and document
  • Work with and assist residents to connect with next recommended LOC
  • Coordinate VOBs with recommended LOCs
  • Ensure residents have plans to transfer to the appropriate LOC and document
  • Ensure residents have scheduled intakes/appointments with the next level of care, or document issues
  • Maintain appropriate documentation on the resident’s follow-through with recommendations
  • Coordinate travel arrangements with resident, ST, and/or referral destination
  • Get resident signature on Continuing Care form

Communication of Information

  • Upload the continuing care form into HCS
  • Communicate recommendations to residents and document
  • Communicate recommendations to family, referent, and TPS and document
  • Communicate challenges and CC plans to treatment team
  • Communicate CC plans to UR/BO
  • Provide continuing care form to resident prior to discharge
  • Include written safety plan with discharge materials
  • Provide continuing care form to family, referent, and TPS as appropriate

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

  • Bachelor's degree in social work, counseling, or related human service field required
  • Preferred: two or more years of experience working in the behavioral health field
  • Previous experience with residential behavioral health setting, case management or utilization review preferred
  • Must be able to pass a criminal background check and drug screen

LICENSES/DESIGNATIONS/CERTIFICATIONS:

  • CPR and CPI/NCI certification (training available upon hire)
